Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Histologic Assessment of Wilm\'s Tumor Farzaneh Jadali Ahmad Mofrad Wilm's tumor is the 4th common malignant tumor of childhood (after CNS tumors, Non Hodgkin's lymphoma and neuroblastoma), and indeed comprises about 5% of childhood malignancies. This is a retrospective study of histologic findings of 23 cases of wilm ' s tumor referred to Mofid Children's Hospital from 1991-1995. 20 Cases were classified as favorable and 3 cases as unfavorable histology . Correct histologic classification helps better therapeutic planning in these patients. Although no of cases is rather small, incidence of two histologic types are compatible with other countries. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Unilateral Developmental Defect of Mullerian Duct Jahangir Derakhshan Developmental defect of Mullerian duct is a congenital malformation characterized by anomalies in famale upper genital organs. As the anomaly is associated with obstruction of the duct, other congenital anomalies of urinary tract specially ipsilateral ranal agenesis are common. This anomaly is often discovered in adolescent period and is usually asymptomatic until puberty. To discover this anomaly as soon as possible, whenever renal agenesis is associated with pelvic mass, Mullerian duct obstructive anomaly should be in mind and ruled out. Our patient was a 15-years-old female with cheif complaint of abdominal and pelvic pain off and on. After Complete work up a diagnostic laparatomy was performed which revealed a double uterus associated with an obstructed hemivagina and ipsilateral renal agenesis. Combination of these anomalies were compatible with diagnosis of unilateral Mullerian duct defect. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Urinary Tract Aspergillosis Following Renal Transplantation Ali Zareie Mahmood Abadi Majid Zarrin The urinary tract aspergillosis is one of the opportunistic mycois, which is secondary to immunocompromised host and transplantation, The etiologic agents, Aspergillus Spp., have been shown to be a common saprophyte of environment. In this case report, a 29 year old man patient with urinary tract aspergillosis was studied. The fungal infection develoed 2 months after renal transpantation. The clinical symptoms were mild and restricted with light fever, weekness and anorexia. Aspergillus f1avus was discovered as the agent responsible for urinary tract aspergillosis by means of direct smear and urine culture , but the results of direct smear and culture of blood and sputum were negative. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Methotrexate in the Treatment of Juvenile Rheumatoid Arthritis Nahid Shafaei Juvenile Chronic Arthritis (JCA) and Juvenile Rheumatoid Arthritis (JRA) are a group of heterogenous idiopathic arthritis with an unclear pathogenesis, most probably, autoimmune phenomenon. The goals of chemotherapy in JRA are to suppress chronic synovitis, to control the systemic effects of the disease on growth and developement of the patients and, ideally, to make young patients lead a normal life. MTX. has proved to be the most effective drug in this therapeutic measures of these patients, but the side effects of the drug are potentially serious and the most important cause for concern the tratment of chi/ren and young adults. Although the mechanism of action of MTX is not fully known, it is said to modify the cytokine synthesis, and with the low dose, increase the adenosine release. In this article we have reviewed the effect of MTX on JCA and JRA during the past 10 years. The results show wide use of MTX. on pediatr c rheumatology patients. For the best results the drug should be used within two years after the diagnosis. The therapy should conform to the protocol of the Pediatric Rhematology Collaborative Study Group (PRCSG). The suggested dose is 10 mg/m2/ week orally. Although the side effects are potentially serious, the use of this drug, under certain considerations is acceptable. Folic acid may decrease some of the side effects. Dehydration, for any reason, and the use of cotrimoxazole should be avoided. When the disease comes under control, the drug can be prescribed every other week. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Evaluation of Early Hypothyroidism Effects on the Gonads Activity Hamid Reza Sadeghipoor Roodsari Farzaneh Ghaderi This study was designed to evaluate specific effect of thyroid hormone on growth and development of reproductive organs in male rats, during one year period from 1994-1995 in the Dept of physiology M.S.U. of Tehran. In this investigation the early hypothyroidism was induced with the use of PTU and the developmental changes in endocrine and nonendocrine organs (kidney and spleen) were studied. The test group animals were those who recieved PTU, and the second group were the control animals. During different periods after birth serum levels of testosterone hormones, testicular total protein synthesis, body weight and the ratio of spleen and kidney weights to total body weights were measured and compared with control groups. In conclusion, in test group weight increase rate up to 60th day of life were less than the control and the differences were statistically significant (p<O.OO1). PTU effects on non endocrine system, weight increases (kidney and spleen) were not significant. Testicular weight up to the 60th day in the test group showed a significant difference when compared to controls in the test animals. The total testicular protein synthesis and testosterone secretion were less than controls, too. We conclude that the thyroid hormones play an effective role in regulation and control of testis and their absence cause significant changes in gonads, hypophysis and probably hypothalamus secretory patterns, with the lag in onset of puberty, in male rats. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Intraosseous Ganglion of the Scaphoid Ebrahim Ameri Although soft tissue ganglia are commonly encountered, their intraosseous counterparts have been rarely recorded. This apparent rarity may be due to confusion in terminology or asymptomatic lesions. The intra osseous ganglion is grossly and histologically identical to soft tissue ganglion and has the same pathogenesis. The patient has mild pain during activity or may be asymptomatic. Lab tests normal. These lesions present a characteristic lytic roentgenographic appearance with sclerotic rim and are most often seen eccentrically at the end of a long bone, frequently the lower end of the tibia, in middle aged patients. Bone scan demonstrates increased uptake. There are two types of lesions: one apparently arising from penetration of soft tissue ganglion into the underlying bone, and the other is an idiopathic from which is primarily intra osseous. Although the pathogenesis is still uncertain, current opinion tends to support the theory of intramedullay metaplasia and proliferation of fibroblastic elements (fibroplasia): The mechanical stress and repeated minor trauma near the surface of the bone may lead to intramedullary vascular disturbance with consequent foci of aseptic bone necrosis. The revitalisation of these areas causes fibroblastic proliferation, followed by a degenerative stage with mucoid secretion and accumulation which enlarges the ganglionic cyst. Curretage (With or without bone graft) or excision is usually effective, and recurrence is rare. The purpose of this paper is to describe a case of an intraosseous ganglion occurring within the scaphoid bone, and review of the literature. It is abundantly present in human placenta. It could be extracted from this organ to the extent of 1mg/placenta using chemical ion exchange and affinity techniques. Molecular weight of this protein is estimated by electophoresis or SDS-PAG electrophoresis using standards of known proteins. Molecular weight has been found to be 48000D. The inhibitor could from a 1 :1 complex with bovine pancreatic ribpnuclease in a noncompetitive fashion (Ki= 2.9. 10-10). Optimal PH for such inhibition is found to be 7.6±0.1 This compound is currently for nucleic acid separations due to its protective abilities and also in genetic engineering, to identify genetic defects in a wide variety of diseases. Thus, due to the importance of the subject, we undertook the kinetic - inhibitory studies of this inhibitor to obtain more insight about the regulatory mechanism of the protein. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Diagnosis and Classification of Leukemias Ahmad Kazemi Parvaneh Vosough Abdolmajid Moazedi Parvin Shahandeh Fazel Shokri Soodabeh Hosseini Faranak Ahmadieh Azarmidokht Farahbakhsh 221 samples of leukemic patients were studied, using morphologic, cytochemical staining pattern and immunophenotyping criteria. Patients included 63 children and 158 adults. Out of the adult group, 110 cases had acute leukemia including 67% AML, 330'{' ALL. In this group, 24 cases of CML" 19 cases of CLL, 4 cases of HCL and one case of plasma cell leukemia were classified. Out of 63 cases of leukemia in children, 79.4% diagnosed as ALL, 19% AML and 1.6% JCML. Among subgroups of AML in adults, only M4 showed to have significant differences (p= 0.001) with the results from Wester countries. According to FAB criteria, L 1 and L2 morphology were the most common types of ALL, respectively in children and adults. In adults 15.2% of chronic leukemia belonged to CML and 12% to CLL. The comparsion of present results with other studies in Western countries 'Showed the lower incidence of CML and CLL in Tehran. The difference was significant only in CLL. Immunophenotype of all CLL cases was B-CLL and no T-CLL was observed. The most common immunophenotype of ALL was C-ALL (68%) and T cell,ALL (530'{') , respectively in children and adults. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 The Prevalance of Iron Deficiency in Pregnant Women Seyed Zia Latifzadeh Ahmad Kazemi Iron deficiency is the most common form of nutritional deficiency in the world. Among different sex and age groups of population pregnant women and infants seems to be in higher risk to develope iron deficiency. According to recent data about half of the pregnant women suffer from iron deficiency. In order ot investigate the prevalance of iron deficiency and iron deficiency anemia in pregnant women, the level of Hb, Mev, serum ferritin and the percentage of transferrin saturation measured in 417 pregnant women and cord blood of their newborns in eight university hospitals in Tehran. 34.8% of pregnant women showed to have iron defeciency and 12.2% iron defeciency anemia. Only one newborn (0.2%) has iron deficiency. The mean Hb concentration in newborns of anemic mothers was significantly lower than those who were borned from normal mothers (p<0.02) This study indicate that the prevalance of iron deficiency in present population was less than the previously published results both in Iran and neighbouring countries. This trend of improvement could be the result of Iron supplement administration to pregnant women which practiced more regularly in recent years. Razi Journal of Medical Sciences RJMS 2228-7043 2228-7051 1998 5 1 Assessment of Anthropometric Indicators of Children Shahandokht Najm Abadi This study was carried out with the purpose of the evaluation of anthropometric indicators in children under 6 years from the west part of Tehran. The survey method was a descriptive cross sectional study and the sampling method was random strata cluster. The anthropometric data of 890 children under 6 years were recorded in questionnaire and analizid with EPls and SPSS soft ware programs. The survied indicators included. Weight for age, Height for age (Stunting) and weight for height (wasting) in all children under 6 years and chest / head circumference in children under 2 years of ages. The rate of lack in weight for age was 13% in boys and 11.1% in girls. The rate of lack in weight for height (W/H) was 11% in qoys and 12.1% in girls, and the rate of lack in height for age (stunting) was 17.6% in boys and 9.8% in girls. The rate of chest/head circumference in children under 2 years indicated an average lack of 31.3% from 7 to 24 months.
