fa مقایسه یافته های سی تی اسکن در کودکان مبتلا به کوید-19 با PCR مثبت و منفی مراجعه کننده به بیمارستان کودکان حضرت علی اصغر(ع) در سال 1400 Comparison of CT scan findings in children with COVID-19 with positive and negative PCR referred to Aliasghar Children's Hospital in 2021-2022 رادیولوژی Radiology پژوهشي Research زمینه و هدف: سی تی اسکن به عنوان یک ابزار تصویربرداری ساده و سریع، می‌تواند ضایعات ریوی در کودکان مبتلا به COVID-19 را به دقت شناسایی و ارزیابی کند. الگوهای تصویربرداری هنوز یکی از مسائل مهم در استفاده از یافته های CT برای تشخیص بالینی COVID-19 می باشد. در مطالعه حاضر، هدف ما ارزیابی یافته های سی تی اسکن در کودکان مبتلا به کوید-19 با PCR مثبت و منفی مراجعه کننده به بیمارستان کودکان حضرت علی اصغر(ع) در سال 1400 بود. مواد و روش ها: این مطالعه cross sectional بر روی کودکان مبتلا به کوید-19 مراجعه کننده به بیمارستان کودکان علی اصغر در سال 1400 انجام شد. در این مطالعه بیماران به دو گروه تقسیم شدند: (1) بیماران مبتلا به کوید-19 با PCR مثبت، (2) بیماران مبتلا به کوید-19 با PCR منفی. تمام بیماران طی دو روز اول تحت CT قرار گرفتند و سپس اطلاعات مورد نیاز در چک لیستی شامل مشخصات بالینی و یافته های تصویر برداری ثبت گردید. CT اسکن بیماران توسط یک رادیولوژیست Review شد و در نهایت نماهای CT کودکان بر اساس RSNA Structured Reporting 3) به 4 دسته Typical، Atypical، Indeterminate و Negative Category CTs تقسیم شدند. داده ها با استفاده از نرم افزار SPSS نسخه 23 آنالیز شدند و سطح معنی داری کمتر از 0.05 در نظرگرفته شد. نتایج: در مجموع 59 نفر (40.4%)  در سی تی اسکن خود دچار درگیری ریه بودند. (14 نفر در گروه PCRمنفی و 45 نفر در گروه PCR مثبت). اگرچه  درگیری ریه در گروه PCR  مثبت بیشتر از گروه منفی بود (42.5% در مقابل 35%) اما این تفاوت از نظر آماری معنی دار نبود. در مطالعه ما الگوی غالب در بین بیماران PCR مثبت و منفی به ترتیب Consolidation، Mixed و سپس GGO بود.  الگوی غالب در موارد PCR منفی Consolidation بود  در حالیکه  الگوهای اپاسیته شیشه مات ، Consolidation و الگوی Mixed  در گروه PCR مثبت تقریبا به یک اندازه دیده شدند و تفاوت بین دو گروه از این نظر نزدیک به معنی داری بود. اگرچه Sign  های Halo و Round در گروه PCR منفی بیشتر بود تفاوت معنی داری بین این دو گروه در میزان مشاهده Sign  های Halo،  Reverse Halo، Crazy Paving و Round  بین دو گروه مطالعه وجود نداشت. میزان درگیری تیپیک  و اینتر مدیت به طور مساوی (15.9%) و بیشتر از آتیپیک (10.9) در دو گروه پی سی آر منفی و مثبت بود که این الگو مستقل از نتیجه PCR بیماران بود. تیپیک بودن درگیری ریه با شدت بیماری تفاوت معنی داری نشان نداد. مقایسه توزیع اپاسیته در سی تی اسکن ریه نشان داد که درگیری های محیطی در موارد PCR مثبت  به طور معنی داری بیشتر از موارد PCR منفی است و این تفاوت به لحاظ اماری معنی دار است. درگیری دو طرفه ریه در موارد PCR مثبت بیشتر بود اما تفاوت بین دو گروه از نظر آماری معنی دار نبود.  همچینین طبقه بندی ضایعات بین دو گروهPCR مثبت و منفی تقریبا یکسان بود. مقایسه درگیری لوبهای مختلف ریه  تفاوت معنی داری بین دو گروه PCR مثبت و منفی نشان نداد. نتیجه گیری: نتایج مطالعه ما نشان داد، درگیری های محیطی در موارد PCR مثبت بیشتر از موارد PCR منفی بود. الگوی غالب در موارد PCR منفی Consolidation بود  در حالیکه در گروه PCR مثبت الگوهای اپاسیته شیشه مات ، Consolidation و الگوی Mixed  تقریبا به یک اندازه دیده شدند و تفاوت بین دو گروه از این نظر نزدیک به معنی داری بود.   Background: CT scan, as a simple and fast imaging tool, can accurately identify and evaluate lung lesions in children with COVID-19. Imaging patterns are still an important issue in the use of CT findings for clinical diagnosis of COVID-19. In the present study, our aim was to evaluate the CT scan findings in children infected with Covid-19 with positive and negative PCR referred to Ali Asghar Children's Hospital in 2021-2022. Materials & methods: In this cross-sectional study children with covid-19 referred to Ali Asghar Children's Hospital in 2021-2022 included. Patients with Covid-19 were divided into two groups: (1) patients with positive PCR, (2) patients with negative PCR. All patients underwent CT during the first two days of admision, and then the required information was recorded in a checklist including clinical characteristics and imaging findings. The CT scans of the patients were reviewed by a pediatric radiologist, and finally, the CT findings were divided into 4 patterns based on RSNA Structured Reporting 3: Typical, Atypical, Indeterminate and Negative Category CTs). The data were analyzed using SPSS software version 23 and the significance level was considered < 0.05. Results: A total of 59 subjects (40.4%) had lung involvement in their CT scan. (14 in the PCR-negative group and 45 in the PCR-positive group). Although lung involvement was higher in the PCR positive group than the negative group (42.5% vs. 35%), but this difference was not statistically significant. In our study, the predominant pattern among PCR positive and negative patients was Consolidation, Mixed and then GGO respectively. The dominant pattern in PCR negative cases was Consolidation, while GGO, Consolidation and Mixed patterns were seen almost equally in the PCR positive group and the difference between the two groups was close to significance. Although Halo and Round signs were more in the negative PCR group, there was no significant difference between these two groups in terms of Halo sign, Reverse Halo, Crazy Paving and Round signs between study groups. The rate of typical and intermediate involvement was equal (15.9%) and more than atypical (10.9%) in both negative and positive PCR groups, which was independent of the PCR result of the subjects. The typicality of lung involvement did not show a significant difference with the severity of the disease. Comparison of opacity distribution in lung CT scan showed that peripheral involvement in PCR positive cases is significantly more than PCR negative cases and this difference was statistically significant. Bilateral lung involvement was more in PCR positive cases, but the difference between the two groups was not statistically significant. The classification of lesions was almost the same between the two groups. Comparing the involvement of lung lobes showed no significant difference between the two PCR positive and negative groups. Conclusion: our finding showed that peripheral involvements were more in the case of positive PCR than in negative PCR cases.
