fa بررسی پیش آگهی کوتاه مدت عملکرد کلیه در کودکان مبتلا به کووید-19 تحت درمان با رمدسیور Investigation of short-term prognosis of renal function in children diagnosed with COVID-19 treated with Remdesivir کلیه اطفال Pediatric Nephrology پژوهشي Research مقدمه:کروناویروس جدید 2019 (SARS-CoV-2) به عنوان یک پاتوژن انسانی مرتبط با عفونت‌های شدید تنفسی شناخته می‌شود. رمدسیور اولین دارویی است که توسط سازمان غذا و داروی ایالات متحده (FDA) برای استفاده بالینی در بیمارستان های مبتلا به COVID-19 تایید شده است. این مطالعه به بررسی نتایج عملکرد کلیوی بیماران تحت درمان با رمدسیور می پردازد. روش اجرا: این مطالعه کوهورت و آینده نگر در بخش های داخلی بیمارستان حضرت علی اصغر(ع) دانشگاه علوم پزشکی ایران انجام شد. معیارهای ورود عبارت بودند از: سن بین 1 تا 18 سال، نداشتن سابقه قبلی بیماری کلیوی و تشخیص بیماری شدید COVID-19 که برای دریافت رمدسیور اندیکاسیون داشتند. رمدسیور در روزهای 2 تا 5 بستری  با پروتکل  درمانی یکسان به بیماران داده شد. علایم بالینی، الکترولیت‌ها،فاکتورهای التهابی و آزمایش عملکرد کلیوی در روز اول درمان و پس از 12 ماه بررسی شد. یافته‌ها: در این مطالعه 114 بیمار مبتلا به کووید-19 در دو دسته بدون MISC COVID-19 و MISC COVID-19 مورد بررسی قرار گرفتند. مردان در گروه بدون MISC و MISC به ترتیب با 25 مورد (54.3 درصد) و 42 مورد (61.8 درصد) غالب بودند. همچنین در گروه بدون MISC و MISC به ترتیب 19 مورد (41.3%) و 36 مورد (52.9%) زیر 5 سال سن داشتند. بیشترین اندام درگیر در این دو گروه به ترتیب مربوط به دستگاه گوارش با 33 مورد (71.7%) و 58 مورد (85.3%) بود. در دو گروه بدون MISC و MISC COVID-19 به ترتیب 6 مورد (13.0%) و 38 مورد (55.9%) علائم کلیوی AKI مشاهده شد. پس از 1 سال پیگیری، تفاوت معنی‌داری در یافته‌های آزمایش خون بین گروه‌های No MISC و MISC COVID-19 وجود نداشت (05/0>P).یافته‌های غیرطبیعی سونوگرافی، گازهای خون وریدی (VBG) HCO3، هماچوری آزمایش ادرار (U/A) و وجود WBC در آزمایش ادرار به طور معنی‌داری پس از یک سال نسبت به قبل به مقدار نرمال تغییر یافت (05/0>P). در گروه MISC COVID-19، تمام پارامترهای عملکرد کلیوی به طور قابل توجهی به حد نرمال بهبود یافت (05/0>P).یافته های غیر طبیعی سونوگرافی، کراتینین خون، پروتئینوری در U/A و وجود میکرو آلبومین U/A در گروه MISC COVID-19 به طور معنی داری بیشتر از بیماران COVID-19 بدون MISC بود (05/0>P). نتیجه‌گیری: بیماران مبتلا به کووید-19 MISC هنگام دریافت رمدسیور در ابتدای پذیرش، علائم نارسایی حاد کلیه یا AKI بیشتری ایجاد می‌کنند. پس از پیگیری 1 ساله، هیچ تفاوتی در نتیجه COVID-19 بین بیماران MISC و No MISC وجود ندارد. کلمات کلیدی: کووید-19، MIS-C، رمدسیور، AKI، CKD <div aria-label="شکستن صفحه" class="cke_pagebreak" contenteditable="false" data-cke-display-name="pagebreak" data-cke-pagebreak="1" style="page-break-after:always" title="شکستن صفحه"></div> Introduction: This study investigates the renal function outcome of patients treated with Remdesivir. Methods: This cohort and prospective study was conducted in the internal departments of Hazrat Ali Asghar Hospital of Iran University of Medical Science. The entry criteria were: age between 1 to 18 years, no previous history of kidney disease, and diagnosed with severe COVID-19 which were indicated to receive Remdesivir. Remdesivir were administered to the patients at days 2-5 of admission in same protocols. Clinical features, electrolytes, inflammatory, and renal function laboratory test were examined in first day of treatment and after 12 months. Results: In this study, 114 patients with COVID-19 were examined in two categories of No MISC COVID-19 and MISC COVID-19. Men were dominant in No MISC and MISC group with 25 cases (54.3 percent) and 42 cases (61.8 percent), respectively. Also, in No MISC and MISC group 19 cases (41.3%) and 36 cases (52.9%) were under 5 years old, respectively. The most organs involved in these two groups were Gastrointestinal with 33 cases (71.7%) and 58 cases (85.3%) respectively. There were 6 cases (13.0%) and 38 cases (55.9%) of AKI kidney symptoms in the two groups of No MISC and MISC COVID-19 patients, respectively. After 1 year of follow-up, there was no significant difference in blood test findings between the No MISC and MISC COVID-19 groups (P>0.05). Abnormal ultrasound findings, Venous Blood Gas (VBG) HCO3, Urinalysis (U/A) Hematuria and U/A WBC were significantly changed to normal value after one year compared to before (P<0.05). In the MISC COVID-19 group, all renal function parameters were significantly changed to normal limits (P<0.05). Abnormal ultrasound findings, Creatinine, U/A protein and the presence of U/A micro albumin in MISC COVID-19 group were significantly more than No MISC COVID-19 patients (P<0.05). Conclusion: MISC COVID-19 patients develop more AKI symptoms while receiving Remdesivir at admission baseline. After 1-year follow up there is no difference in COVID-19 outcome between MISC and No MISC patients.
