پاسخ ایمنی به واکسن هپاتیت B در بیماران مبتلا به لوسمی لنفوبلاستیک حاد در فاز نگهدارنده شیمی درمانی در بیمارستان حضرت علی اصغر(ع)

<p></p><p> زمینه و هدف: HBV(Hepatitis B virus)، یک ویروس هپاتوتروفیک است که معمولاً سبب عفونت بدون علامت میگردد و یک عامل عفونی بسیار مهم در بیماران مبتلا به لوسمی لنفوبلاستیک حاد به دلیل نیازی که این بیماران به تزریق فراوردههای خونی در حین درمان دارند، میباشد. با توجه به اهمیت موضوع، هدف از انجام این مطالعه، استفاده از واکسیناسیون هپاتیت B در بیمارانی که نسبت به این بیماری مصونیت ندارد، میباشد. روش بررسی: برای پیشگیری، واکسیناسیون هپاتیت B، با دوز دو برابر، در 159 بیمار مبتلا به لوسمی لنفوبلاستیک حاد در فاز نگهدارنده درمان، انجام شد. بیماران قبل از انجام واکسیناسیون، از نظر سیستم ایمنی(سلولی و همورال) و عملکرد کبدی بررسی شدند و هیچ نوع مشکلــی نداشتنـد. مطالعــه حاضر از نوع موردی ـ شاهدی بود که جهت آنالیز دادهها از آزمون Chi-square استفاده شد. یافتهها: نتایج بررسی HbsAb(Hepatitis B surface Antibody) در هر دو گروه، 3 الی 5 ماه بعد از واکسیناسیون پیگیری شد که حدود 25% از بیماران پاسخ مثبت به HbsAg(Hepatitis B surface Antigen) دادند و این یک نشانه امیدوار کننده برای بررسی بیشتر و کنترل بیشتر در بیماران است. نتیجهگیری: مطالعات انجام شده در کشورهای دیگر نشان دهنده این است که حدود 30%، پاسخ مثبت به واکسن HbsAg دادند. به نظر میرسد که بهتر است تزریق واکسن هپاتیت B در بیمارانی که سطح آنتیبادی محافظتی در برابر این بیماری ندارند، انجام گردد. </p>
<p></p><p> Background & Aim: HBV is hepatotrophic virus that usually causes infection without any symptoms and it is a very important infective factor in patients with acute lymphoblastic leukemia because they need blood products during their treatment. Because of the importance of this subject the aim of this study was to use hepatitis B virus vaccination in patients who did not have immunity for this disease. Patients and Methods: We used double dose hepatitis B virus vaccination for prevention of hepatitis B in 159 patients with acute lymphoblastic leukemia that were in maintenance phase of chemotherapy. Patients immunity system(humoral and cellular) and hepatic function was checked and didn't have any problem. This was a case – control study for which Chi-Square Test was used to analyse data. Results: The results(Hepatitis B Surface antibody) were checked in both groups 3-5 months after vaccination. 25% of patients showed affirmative response to vaccination and it was a hopeful sign for increasing researches and control in these patients. Conclusion: Studies in other countries, show about 30% positive response to HbsAg vaccination and we can conclude that, it is better to use hepatitis B virus vaccination for those who don't have any protective antibody for hepatitis B disease. </p>
