<p></p><p> زمینه و هدف: تأمین منابع خونی سالم و کافی که بتواند نیازهای فیزیولوژیک افراد در حین عمل را فراهم کند، مشکلی جدی و قدیمی میباشد. یک متخصص بیهوشی باید دانش کافی پیرامون اندیکاسیونها و عوارض انتقال خون داشته باشد. روش به کار گرفته شده در این مقاله نورموولمیک همودیلوشن حاد است که روشی توصیه شده و مورد تأیید میباشد و احتمالاً بسیاری از عوارض تزریق خون همولوگ با آن قابل پیشگیری است(نظیر واکنشهای آلرژیک، انتقال عفونتها، ...). هدف از این مطالعه تعیین اثرات و عوارض و مزایای احتمالی آن نسبت به تزریق خون همولوگ می باشد. روش بررسی: در یک مطالعه کارآزمایی بالینی، بیماران ASA(American Society of Anesthesiology) یک و دو که تحت عمل جراحی هیپ و فمور به صورت الکتیو قرار میگرفتند در دو گروه 30 نفره بررسی شدند(در جمع 60 نفر). روش القاء بیهوشی، نگهدارنده آن، فرد اینتوبه کننده، نوع محلولهای مصرفی در دو گروه، مشابه بود. کیسههای خون پس از القاء بیهوشی توسط مسیر شریانی(Arterial line) به دست آمد. در مطالعه صورت گرفته تعداد ضربان قلب، متوسط فشار خون شریانی، میزان اشباع خون شریانی، میزان دیاکسید کربن جریان بازدمی، برون ده قلبی قبل از القاء بیهوشی، یک و دو ساعت بعد از شروع عمل و در انتهای عمل سنجیده شد و در دو گروه مقایسه گردید. حجم خونریزی در پایان عمل در دو گروه مقایسه شد. تعداد کیسه خونهای مصرفی نیز ثبت گردید. یافتهها: تعداد ضربان قلب دو ساعت بعد، با یکدیگر اختلاف آماری معنیداری داشت(001/0=P). همچنین در پایان عمل هم به طور معنیداری در گروه اتولوگ بالاتر بود(001/0=P). متوسط فشار خون شریانی دو ساعت بعد و در پایان عمل در گروه اتولوگ به طور معنیداری بالاتر بود(005/0 & 02/0=p) میزان دیاکسید کربن جریان بازدمی یک ساعت، دو ساعت و در پایان عمل به طور معنیداری در گروه اتولوگ پائینتر از گروه همولوگ بود(00/0، 04/0، 05/0=p). حجم خونریزی در پایان عمل به طور معنیداری در گروه اتولوگ پائینتر بود(00/0=p). نتیجهگیری: با توجه به نتایج حاصله، به نظر میرسد تعداد ضربان قلب در گروه اتولوگ پس از اخذ کیسههای خون بالاتر از گروه همولوگ که این استرس را نداشتهاند بوده است ولی علیرغم این مسئله تغییر قابل ملاحظه و افزایشی در برون ده قلب(بر خلاف انتظار) اتفاق نیفتاده است و دو گروه تقریباً میزان برون ده قلبی مشابهی داشتهاند. هم چنین متوسط فشار خون شریانی و دیاکسید کربن جریان بازدمی در دو گروه متفاوت و در گروه اتولوگ رضایتبخشتر بود. حجم خونریزی هم در گروه اتولوگ به وضوح کمتر از گروه همولوگ بوده است. پس به نظر میرسد کنترل همودینامیک با روش اتولوگ مناسبتر و بهتر از همولوگ باشد. </p>
<p></p><p> Background & Aim: Preparation of safe and enough blood products which can solve the physiologic needs during surgery is a big and old problem. The anesthetist must have adequate knowledge about indications and complications of transfusion. The method used in this study was acute normovolemic hemodilution which is a recommended procedure in reference books. The goals of this study were to determine the effects, benefits and side effects of this method. Patients & Methods: In a randomized clinical trial(RCT), 60 patients were evaluated in two groups of 30. All of them were ASA 1, 2 and between 15-45 years old who were candidates for elective orthopedic surgery. The induction and maintenance routs of anesthesia and used solutions were similar in two groups and all patients were intubated with one person. Blood bags were achieved from arterial line. Heart rates, mean arterial pressure, SPO2, ETCO2 and cardiac output(non invasive with NICO) were measured and recorded before induction, 1 hour later, 2 hours later and at the end of the surgery. Volume of blood loss was calculated at the end of surgery. The number of used blood bags in two groups was recorded. Results: Number of heart rates 2 hours later and at the end of operation was statistically significant and was higher in autologous group(P=0.001, 0.00). Mean arterial pressure 2 hrs later and at the end of the surgery was statistically significant and different in two groups and were higher in autologous group(P=0.02, 0.005). ETCO2 was significantly different 1, 2 hrs later and at the end of the surgery and was lower in autologous group(P=0.005, 0.004, 0.00). Blood loss volume was statistically different in two groups and was lower in autologous group(P=0.00). Cardiac output was not statistically different in two groups. Conclusion: These results show that increase in heart rates after blood donation is probably due to induced stress in autologous group, but no change in cardiac output occurred. Also, mean arterial pressure and ETCO2 were different in two groups and were more satisfactory in autologous group. Furthermore, volume of blood loss was significantly lower in autologous group than hemologous group. It seems that autologous blood transfusion is a better method with more adequate hemodynamic control than hemologous transfusion. </p>
