تعیین و مقایسه میزان پایایی تأثیر درمانی دو روش Ultra Sound تحت استرچ و Hold-Relax بر کاهش کوتاهی عضله همسترینگ

<p style="DIRECTION: rtl" dir="rtl"><strong></strong></p><p style="DIRECTION: rtl" dir="rtl"><strong> زمینه و هدف: </strong>تاکنــون تأثیــر درمانی دو روش US ( Ultra Sound ) تحت استــرچ(کشــش) و Hold-Relax بر کاهش کوتاهی عضلات و تاندونها با هم مقایسه نشده، همچنین تأثیر امواج اولتراسوند نیز بر دوام اثر درمانی استرچینگ مشخص نشده است. در این تحقیق به مقایسه پایایی و دوام تأثیر درمانی دو روش us تحت استرچ و Hold-Relax بر کاهش کوتاهی عضله همسترینگ پرداخته شد. </p><p dir="rtl"><strong>روش کار: </strong>این مطالعه به صورت آزمون بالینی تصادفی( clinical trial ) بر روی 20 زن جوان سالم و غیر ورزشکار با میانگین سنی 32/1 ± 62/21 سال که دچار کوتاهی عادتی دو طرفه عضلات همسترنیگ بودند ( ˚ 65 SLR ≤ : Straight Leg Raising )، انجام شد. نمونهها به صورت تصادفی به 2 گروه 10 نفره تقسیم شدند. درمان در گروه اول به روش US تحت استرچ و در گروه دوم به روش Hold-Relax به مدت 4 هفته و هر هفته 3 جلسه انجام شد. در هر بیمار متغیرها قبل از شروع درمان و بلافاصله بعد از اتمام دوره درمان اندازهگیری شدند، این متغیرها شامل دامنه SLR فعال ( Active ) و غیرفعال( Passive ) ، دامنه اکستنسیون فعال و غیرفعال زانو، تیلت لگن و لوردوز کمر بودند. بعد از اتمام دوره درمان برای پیگیری پایایی درمان ارزیابی تمام متغیرهای فوق در طی 4 مرحله با فواصل 1 هفته انجام شد. </p><p dir="rtl"><strong>یافتهها: </strong>در هر دو گروه درمانی بعد از اتمام دوره درمان همه متغیرها به جز لوردوز کمر تأثیرات درمانی قابل توجه و معنیداری نشان دادند. تأثیر روش درمانی US تحت استرچ بر روی تمام متغیرها نسبت به روش Hold-Relax بیشتر بود، که این برتری در مورد متغیرهای SLR فعال و غیرفعال در پای چپ نمونهها و تیلت لگن نیز معنیدار بود. تأثیر روش درمانی Hold-Relax در اکثر متغیرها تا انتهای هفته چهارم پیگیری پایدار باقی ماند و کاهش معنیداری نشان نداد <strong>.</strong> در حالی که در روش درمانی US تحت استرچ در بیشتر متغیرها از همان هفته اول پیگیری، کاهش معنیدار مشاهده شد. </p><p dir="rtl"><strong>نتیجهگیری کلی: </strong>بنابراین به نظر میرسد باید برای کسب نتایج درمانی حاد و کوتاه مدت از روش US تحت استرچ و برای داشتن پایایی بیشتر از روش Hold-Relax استفاده شود. </p>
<p><strong></strong></p><p><strong> Background & Objective:</strong> The effect of simultaneous US(Ultra-Sound) with stretching, however, has not been compared with other manual stretching techniques like PNF(Proprioceptive Nueromuscular Facilitation). Furthermore, the permanency of the two techniques has not been investigated, either. </p><p><strong>Method: </strong>This study was a randomised clinical trial on 20 non-athlete and healthy women. The subjects were between 18-26 years of age with habitual bilateral hamstring shortness(SLR). They were randomly assigned to two groups consisting of 10 subjects each. The first group received simultaneous US with stretching and the second one received hold–relax treatment for 4 weeks three times per week. Before and immediately after treatment period, these variables were measured: passive and active SLR, passive and active knee extension, pelvic tilt and lumbar lordosis. After the last session of treatment, all the variables were followed up every week, for 4 weeks. </p><p><strong>Results: </strong>Both groups showed a significant improvement in all variables except lumbar lordosis. In comparison to hold-relax technique simultaneous US with stretching showed greater effectiveness. Moreover, this efficacy was significant in variables of left leg SLR, left knee extension and pelvic tilt. Comparing the permanency of the two treatment techniques, it was revealed that the treatment results of PNF were maintained up to the end of the 4th week of follow-up and didn’t decrease significantly, while those of simultaneous US with stretching showed a significant decrease from the first week of follow-up. </p><p><strong>Conclusion: </strong>These findings suggest that simultaneous US with stretching and hold-relax are effective techniques for gaining acute treatment results and more stable results respectively. </p><p> </p>
