بررسی عوامل خطر در عفونت زخم جراحی

<p style="DIRECTION: rtl" dir="rtl"> عفونت زخم جراحی دومین علت شایع عفونت بیمارستانی است که از عوامل مرگ و میر است و هزینههای اضافی ناشی از اقامت طولانی در بیمارستان را موجب میشود. پژوهش حاضر با هدف بررسی عفونت زخم جراحی در بخشهای مختلف و عوامل مستعدکننده و خطر این عفونتها در بیمارستانهای دانشگاه علوم پزشکی ایران انجام شده است. بررسی بر روی 960 بیمار بستری شده در بخشهای جراحی عمومی، زنان، ارتوپدی، گوش و حلق و بینی و جراحی مغز و اعصاب انجام شد که در بیمارستانهای عمومی و تکتخصصی تحت جراحی قرار گرفته بودند. برای بیماران بستری پرسشنامهای شامل موارد منطبق با اهداف طرح توسط دستیاران تکمیل میگردید. بیماران در دو گروه با عفونت زخم و بدون عفونت بررسی شدند. نتایج با استفاده از نرمافزار Epi 6 استخراج شد و از آزمونهای T-Test ، Chi Square و Odds ratio برای تحلیل دادهها استفاده شد. نتایج نشان داد که از میان عوامل مستعد کننده به ترتیب سوء تغذیه، عفونت در نقطهای از بدن، دیابت، چاقی و مصرف سیگار بیشترین احتمال بروز عفونت زخم جراحی را به همراه داشتند. سایر عوامل با احتمال کمتر نیز بررسی شدند. عوامل تکنیکی مرتبط با نوع جراحی، محل و عضو جراحی شده و نحوه درمان پیشگیرانه نیز در دو گروه بیماران دارای عفونت و بدون عفونت بررسی شد. در پژوهش حاضر، نرخ عفونت زخم جراحی 4/8% برآورد شد که برابر با آمار جهانی شیوع زخم جراحی(34-5%) بود. اهمیت هر یک از عوامل مستعدد کننده در بروز عفونت زخم مورد تأکید بیشتر قرار گرفت. </p><p dir="rtl" ><strong> </strong>
<p></p><p> Surgical wound infection is the second most common cause of nosocomial infection which leads to mortality and extra expenses due to prolonged stay in the hospital. The aim of the present study was to survey surgical wound infection in different wards of Iran University Hospitals and investigate the predisposing and risk factors. Investigation was done on 960 patients who underwent surgery in general surgery, gynecology, orthopedic, ENT and neurosurgery wards. For each patient a questionnaire was filled by residents. Patients were categorized into two groups: those with infection and those without evidence of infection. Data were collected by EPI6 and the results were analyzed by t-test, chi-square and odds ratio tests. The results showed that malnutrition, infection in other body sites, obesity and smoking are among the predisposing factors which carry high probabilities of infection. Other factors had low probability. Technical factors pertinent to the type of operation, the site and the organ, prophylactic antibiotic treatment were also studied. Surgical wound infection rate in this study was estimated as 8.5% which is equal to international reports(5-34%). The importance of each predisposing factor was also emphasized in this study. </p><strong> </i /></strong>
