fa بیماری هوچکین و بررسی نتایج درمانی و میزان بقای آن خون و انکولوژی Hematology & oncology پژوهشي Research <p style="DIRECTION: rtl" dir="rtl">در هر سال نزدیک به 7500 مورد بیماری جدید هوچکین در ایالات متحده امریکا شناخته می‌شود. این بیماری از جمله تومورهایی است که تقریباً 75% بیماران مبتلا به آن با شیمی‌درمانی و رادیوتراپی پیش‌رفته بهبود می‌یابنــد. هدف از ایــن مطالعـه افزودن بر اطلاعات موجود در مورد بیماری، استفاده از روش‌های درمانی مختلــف و تعییــن میزان بقای کلی 5 و 10 ساله بوده است. این مطالعه گذشته‌نگر به روش توصیفی ـ تحلیلی ـ مقطعی روی بیماران مبتلا به هوچکین در مرکز آموزشی درمانی قاضی طباطبایی تبریز انجام شد. اطلاعات مورد نظر از پرونده‌های بیماران خارج گردید و میزان پاسخ به درمان، میزان بقای عاری از بیماری( DFS = Disease Free Survival ) و میزان بقای کلی ( OS = Overall Survival ) محاسبه شـد و داده‌ها با استفاده از نرم‌افزار 10 SPSS و آزمون Chi-square مورد تجزیه و تحلیل قرار گرفت. براساس نتایج به دست آمده 121 بیمار(67%) مرد و 59 بیمار(33%) زن بودند. سن بیماران با میانگین 1/17 ± 8/31 سال عامل تاثیرگذار بر میزان بقا نبود. وجود توده در گردن(40%) از شایع‌ترین شکایت‌های بیماران در زمان مراجعه بود که اغلب در مرحله III قرار داشتند و Mixed Cellularity ( MC ) با 2/47% بیش‌ترین زیر گروه هیستولوژیکی را تشکیل می‌داد. رژیم شیمی‌درمانی ABVD با 6/37% روش درمانی شایع به کار رفته با بالاترین میزان بازگشت( Remition ) بود. میزان بقای کلی 5 ساله 65% و میزان بقای کلی 10 ساله 3/61% به دست آمد. در این مطالعه بیمارانی که تحت درمان با شیمی درمانی بودند بیش از سایر روش‌های درمانی(7/52%) بازگشت کامل داشتند و بیماران درمان شده با رژیم‌های دارویی ABVD و MOPP-C به ترتیب میزان بقای 5 و 10 ساله بالایی را نشان دادند. در نهایت می‌توان گفت که تشخیص زودرس، ارزیابی به موقع بیماران و استفاده از روش صحیح درمانی میزان بقای خوبی را برای بیماران فراهم می‌کند. </p><p dir="rtl" > <p></p><p></p><p>Nearly 7500 new cases of Hodgkin’s disease are diagnosed in the United States annually. This disease is one of the first malignancies and can be cured in about 75% of cases. The main objective of this study was to create a major sight on Hodgkin’s disease, the result of its treatment and its 5 and 10-year survival by considering this point that early diagnosis, on time management of the patients and appropriate treatment will give significant survival to these patients. This retrospective, descriptive, analytical and cross-sectional study was performed in Tabriz Shahid Ghazi Hospital. The needed information was extracted from the patients’ files and specific forms were prepared for gathering data. Response to treatment, DFS (Disease Free Survival) and OS (Overall Survival) were calculated and data was analyzed using SPSS10 software and chi-square test. Of total 180 cases, 121 (67%) were male and 59 (33%) were female. The main age range was 31.8 ±17.1 years. Neck mass (40%) was the common complaint of diagnosis and most of the cases were in stage III. Mixed cellularity (47.2%) was the most common histological subtype. Complete remission with ABVD chemotherapy regimen was prominent and it composed 37.6% of the used chemotherapy regimen. Five and ten-year survival were estimated 65% and 61.3% respectively. The results showed that the occurrence of the Hodgkin’s disease was double in males than in females. Patients under chemotherapy had more favorable complete remission (52.7%) than those treated with other forms of treatment (P=0.002). A significant statistical difference was noted between the used chemotherapy regimen and the rate of complete remission (P=0.04). As a whole, 5-year overall survival was higher in patients treated with ABVD, whereas 10-year survival showed to be higher in cases received C-MOPP regimen.
