fa به‌کارگیری رگرسیون لجستیک بیزی برای تعیین عوامل خطر رتینوپاتی دیابتی Application of Bayesian logistic regression for determining the risk factors of diabetic retinopathy آمار زیستی Biostatistics پژوهشي Research زمینه و هدف: دیابت یکی از شایع‌ترین بیماری‌های مزمن قرن حاضر است. پیامدهای متعددی در بیماران دیابتی رخ می‌دهد که از مهم‌ترین آن‌ها را می‌توان رتینوپاتی و ماکولاپاتی دانست. در مطالعه حاضر به بررسی عوامل خطر رخداد رتینوپاتی دیابتی با استفاده از رگرسیون لجستیک بیزی پرداخته شده است. روش کار: در این مطالعه مقطعی-تحلیلی، تعداد 623 بیمار به روش نمونه‌گیری خوشه‌ای چندمرحله‌ای از بین تمامی بیماران دیابتی شهر تهران انتخاب شدند. متغیرهایی همچون سن، جنسیت، طول مدت ابتلا به دیابت، شاخص توده بدنی، هموگلوبین گلیکوزیله، فشار خون سیستولیک و ... در این افراد اندازه‌گیری شده و با استفاده از معاینات چشم وجود رتینوپاتی بررسی گردید. در این مطالعه برای برازش مدل رگرسیون لجستیک بیزی از نرم افزار SAS نسخه 9.3 استفاده شد. یافته‌ها: در مطالعه حاضر تعداد 623 بیمار دیابتی مورد بررسی قرار گرفتند که از این میان، 4/54% (339 نفر) نمونه را زنان و 6/45% (284 نفر) را مردان تشکیل داده‌اند. در حدود 38% (236 نفر) از این بیماران دیابتی مبتلا به رتینوپاتی بوده و میانگین ( gte msEquation 12]><m:omath><m:r><m:rpr><m:scr m:val="roman"><m:sty m:val="p"></m:sty></m:scr></m:rpr>±</m:r></m:omath> !msEquation]--><v:shapetype coordsize="21600,21600" filled="f" id="_x0000_t75" o:preferrelative="t" o:spt="75" path="m@4@5l@4@11@9@11@9@5xe" stroked="f"> <v:stroke joinstyle="miter"> <v:formulas> <v:f eqn="if lineDrawn pixelLineWidth 0"> <v:f eqn="sum @0 1 0"> <v:f eqn="sum 0 0 @1"> <v:f eqn="prod @2 1 2"> <v:f eqn="prod @3 21600 pixelWidth"> <v:f eqn="prod @3 21600 pixelHeight"> <v:f eqn="sum @0 0 1"> <v:f eqn="prod @6 1 2"> <v:f eqn="prod @7 21600 pixelWidth"> <v:f eqn="sum @8 21600 0"> <v:f eqn="prod @7 21600 pixelHeight"> <v:f eqn="sum @10 21600 0"> </v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:f></v:formulas> <v:path gradientshapeok="t" o:connecttype="rect" o:extrusionok="f"> <o:lock aspectratio="t" v:ext="edit"> </o:lock></v:path></v:stroke></v:shapetype><v:shape id="_x0000_i1025" style="width: 7.2pt height: 13.2pt" type="#_x0000_t75"> <v:imagedata chromakey="white" src="file:///C:UsersKatyAppDataLocalTempmsohtmlclip11clip_image001.png"> </v:imagedata></v:shape>انحراف معیار) سن زنان و مردان به ترتیب برابر 05/11±5/59 و 65/11±5/60 سال بود. با توجه به نتایج مدل رگرسیون لجستیک بیزی، متغیرهای سن، طول مدت ابتلا به دیابت، هموگلوبین‏ گلیکوزیله، جنسیت، نوع انسولین مصرفی، ابتلا به ادم ماکولار و فشارخون تاثیر آماری معناداری در بروز بیماری رتینوپاتی داشتند. نتیجه‌گیری: در تعیین عوامل خطر رتینوپاتی دیابتی، نتایج رگرسیون لجستیک بیزی و مدل لجستیک کلاسیک از نظر جهت و اندازه تقریبا یکسان بوده ولی برآوردهای بیزی بازه‌های اطمینان کوتاه‌تری دارند. Background: Diabetes is one of the most common chronic diseases of this century. Retinopathy and makulopati are two most important implications of diabetes. In this study, Bayesian logistic regression is used to assess the factors affected on diabetic- retinopathy. Methods: Study population of this cross-sectional study contains all diabetic patients in Tehran of which 623 of them were selected using multi-stage cluster sampling. Age, BMI, hemoglobin, cholesterol, hypertension, duration of diabetes, etc were measured and the status of diabetes were assessed. Bayesian logistic regression was applied using SAS 9.2 software. Results: Of 623 diabetes patients, 54.4% (339 patients) were female and 45.6% (284 patients) were male. In 38% (n=236) of patients diabetic retinopathy were occurred and mean (±SD) age of females and males were 59.5±11.05 and 60.5±11.65 years, respectively. Using Bayesian logistic regression, statistically significant associations were revealed between diabetic retinopathy and age, sex, type of insulin, duration of diabetes and macular edema.
