fa پایلوت شبکه هشدار سوء مصرف مواد در بیماران مسموم Substance abuse warning network: Pilot results in poisoned patients اپیدمیولوژی Epidemiology پژوهشي Research   زمینه و هدف: پدیده طبی-اجتماعی سوء مصرف مواد یکی از بحران هایی است که جامعه ما امروزه با آن مواجه است. مطالعه پیش رو نتایج پایلوت شبکه هشدار را در بیمارستان لقمان حکیم تهران نشان می دهد.   روش کار:<span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw=""> بعد از مصاحبه با 1850بیمار مراجعه کننده به اورژانس مسمومین در بیمارستان لقمان حکیم در طول 12 ماه که به شکل اتفاقی انتخاب شده بودند، 16 ماده مورد سوء استفاده شامل مورفین، متادون، ترامادول، بوپرنورفین، اکسی کودون، پروپوکسی فن، آمفتامین ، متامفتامین، کوکائین، 3و4 متیلن دی اکسی متامفتامین، حشیش، کتامین، فن سیکلیدین، بنزودیازپین، باربیتورات و الکل به شکل اتفاقی با کمک کیت های ایمنو اسی مورد بررسی قرار گرفتند.   یافته ها:<span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw=""> مجموعاً 1258 نفر (68% <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: red line-height: 150% font-family: " tahoma="" zh-tw="">از جمعیت هدف<span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">) شامل 739 زن (7/58%) حاضر به ورود به مطالعه شدند تا مورد پایش سوء مصرف و مصاحبه در اورژانس قرار گیرند. متوسط سنی11± 28 ( محدوده 88-12 سال) بود. 945 بیمار (1/75%) عمداً خود را مسموم کرده، در حالی که مسمومیت با مواد علت پذیرش در 194 بیمار (4/15%) بود. حداقل تعداد کل بیماران سوء مصرف کننده مواد اوپیوئیدی 397 نفر (6/31%) بود که 144 نفر<span 12pt="" b="" dir="ltr" fa="" font-size:="" lang="FA" mso-bidi-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " zh-tw=""> <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">(4/11%) سوء مصرف کننده پنهانی بودند که با آزمایش ها  مشخص گردیدند. مواد محرک 123 نفر (8/9%) با 49 نفر (9/3%)<span 12pt="" b="" dir="ltr" fa="" font-size:="" lang="FA" mso-bidi-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " zh-tw=""> <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">جمعیت پنهان، مواد    <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">توهم زا 22 نفر (7/1%) با 15 نفر (2/1%)<span 12pt="" b="" dir="ltr" fa="" font-size:="" lang="FA" mso-bidi-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " zh-tw=""> <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">جمعیت پنهان، مواد سداتیو 337 نفر (8/26%) با 145 نفر (5/11%)<span 12pt="" b="" dir="ltr" fa="" font-size:="" lang="FA" mso-bidi-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " zh-tw=""> <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">جمعیت پنهان و الکل با 76 نفر(6%) مواد دیگر بودند. <span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw="">  نتیجه گیری:<span 12pt="" b="" fa="" font-size:="" lang="FA" mso-ascii-font-family:="" mso-bidi-language:="" mso-fareast-language:="" mso-hansi-font-family:="" mso-no-proof:="" style="color: black line-height: 150% font-family: " tahoma="" zh-tw=""> تعداد قابل توجهی سوء مصرف پنهان مواد در جامعه وجود دارند که نیازمند مداخله فوری و مناسب هستند.      Background: The sociomedical phenomenon of drug abuse remains among the most critical issues facing our society, today. The present study reports the pilot results of Drug Abuse Warning Network in Loghman-Hakim Hospital,Tehran.   Methods: After taking history of 1850 patients who were chosen randomly  for interview  and screened for drug of abuse in the emergency department during 12 months16 substances including morphine, methadone, bupernorphine, oxycodone, tramadol, propoxyphene, amphetamine, methamphetamine, 3, 4- ethylenedioxy methamphetamine, cocaine, ketamine, phencyclidine, tetrahydrocanabinol, benzodiazepines, ethanol and barbiturates were evaluated using urine screening immunoassay kits in Loghman-Hakim Hospital.    Results: Totally 1850 patients were chosen randomly  for interview  and screened for drug of abuse in the emergency department.  From the above group 1258 (68%) including 739 (58.7%) women accepted to enter the study. The mean age was 28±11 (range 12-88). Totally 945 (75.1%) patients had deliberately poisoned themselves, while substance intoxication was the cause of admission in 194 (15.4%) patients. Taken all minimum substance abusers together, opioid abusers were 397 patients (31.6%), of whom 144 cases (11.4%) were hidden abusers that were revealed by lab tests.
