fa بررسی رابطه میان آنتی ژن اختصاصی پروستات و نمایه توده بدنی در بیماران با علائم انسداد ادراری Relationship between body mass index and prostate specific antigen in patient with lower urinary tract symptoms ارولوژی Urology پژوهشي Research زمینه و هدف: در سال‌های اخیر استفاده از تست سطح سرمی آنتی ژن اختصاصی پروستات (Prostate Specific Antigen-PSA) برای تشخیص سرطان پروستات، سبب کاهش مرگ و میر ناشی از آن شده است. برخی عوامل با تأثیر بر سطح سرمی PSA سبب تاخیر در انجام بیوپسی پروستات و یا انجام غیر ضروری آن می‌شوند. برخی مطالعات اثر وزن بدن و نمایه توده بدنی Body Mass Index-BMI)) بر PSA را تایید کرده‌اند و برخی دیگر نظر متناقضی دارند. در مطالعه حاضر به بررسی ارتباط بین نمایه توده بدنی و PSA پرداخته شده است. <o:p /> روش کار: در مطالعه مقطعی حاضر به بررسی 140 بیمار که با شکایت علائم ادراری به درمانگاه بیمارستان امام رضا (ع) دانشگاه علوم پزشکی مشهد مراجعه کرده بودند، پرداخته شد. بیمارانی که مبتلا به سرطان پروستات، پروستاتیت، عفونت ادراری مکرر، مصرف فیناستراید و سابقه جراحی پروستات بودند، از مطالعه خارج شدند. سپس اطلاعات مربوط به وزن، نمایه توده بدنی، حجم پروستات، PSA و Free PSA این بیماران جمع آوری شد و به دو گروه با نمایه توده بدنی کمتر از 27 (گروه الف) و مساوی و بیشتر از 27 (گروه ب) تقسیم شدند و سپس با کمک آزمون T-test و همبستگی و با کمک نرم افزار SPSS 15 آنالیز شدند. <o:p /> یافته‌ها: میانگین سنی افراد 98/9 ± 6/63 سال بود. در دو گروه مطالعه سن، حجم پروستات و Free PSA تفاوت معناداری نداشت. اما میانگین PSA در گروه BMI کمتر از 27 (41/3 ± 9/3) و در گروه با BMI بالای 27 (31/2 ± 7/2) بود که تفاوت معنادار داشت (p=0.04). همچنین بین PSA و BMI همبستگی معنی دار معکوس موجود بود (p=0.04 , R=-0.175).<o:p /> نتیجه‌گیری: مطالعه حاضر نشان داد که با افزایش نمایه توده بدنی، PSA بیماران کاهش می یابد . اما نمایه توده بدنی بر PSA Free اثری ندارد و نسبت میان PSA آزاد به توتال در بیماران چاق افزایش می یابد و می‌تواند سبب تأخیر در تشخیص سرطان پروستات شود. نتیجه مطالعه حاضر می‌تواند به تعیین Cut point های مختلف بر اساس نمایه توده بدنی برای انجام بیوپسی پروستات با کمک مطالعات بزرگ تر در آینده کمک کند.<o:p /> Background: In recent years the use of Prostate Specific Antigen test (PSA) has reduced the mortality rate of prostate cancer. Certain factors by affecting the levels of PSA, decrease or increase prostate cancer detection. Some studies suggest the effect of body weight and Body Mass Index (BMI) on PSA, while some others are contrary to the opinion mentioned.. Hence, in this study we examined the relationship between BMI and PSA.<o:p /> Methods: In the present study 140 patients with LUTS (Lower Urinary Tract Symptoms) that had referred to the Urology clinic of Imam Reza Hospital (affiliated to Mashhad University of Medical Sciences), were studied. Patients with prostate cancer, prostatitis, recurrent UTI, taking finasteride and history of prostate surgery were excluded. The data of body weight, BMI, prostate volume, PSA and Free PSA were collected. Patients were divided in two groups group A with a BMI less than 27 and group B with BMI equal and greater than 27. The data were analyzed with T-test, Correlation and SPSS (version 15).<o:p /> Results: Mean of age in patients was 63.6 ±9.98.Two groups in regard to age, prostate volume, and Free PSA did not differ statistically. Mean PSA level in group A was 3.9±4.3 and in group B is 2.7±1.3. There is a significant difference in mean PSA level between two groups (p=0.04). We tested the correlation between PSA levels and BMI which were inversely correlated with increased BMI (p=0.04, R=-0.175).<o:p /> Conclusions: This study showed that PSA is reduced with increasing BMI. However BMI does not affect Free PSA, thus the ratio between total PSA and free PSA rises, delaying prostate cancer diagnosis. The result of the present study showed that in different BMI levels physicians need different PSA cut points to decide on performing prostate biopsy.<o:p /> نمایه توده بدنی، آنتی ژن اختصاصی پروستات-سرطان پروستات.
