fa مقایسه‌ی تغییرات سیتومورفومتری مخاط دهان در افراد سیگاری و قلیانی Comparison of cytomorphometry changes in oral mucosa of smoker and hookah users آسیب‌شناسی Pathology پژوهشي Research زمینه و هدف: مخاط دهان افراد سیگاری و قلیانی جهت ایجاد تغییرات بدخیم مستعدتر است. لذا هدف مطالعه حاضر ارزیابی تأثیر سیگار و قلیان بر ویژگی‌های مورفومتری (Morphometry) مخاط طبیعی دهان می باشد.<o:p /> روش کار: در این مطالعه کوهورت گذشته نگر، اسمیر سیتولوژی(Cytology) از 3 ناحیه مخاط باکال، طرفی زبان و کف دهان (سمت راست)، در 40 فرد سیگاری، 40 نفر قلیانی و 40 فرد سالم تهیه شده و به روش پاپانیکولائو <span lang="AR-SA" style="font-family: "Arial","sans-serif" font-size: 10pt mso-ascii-font-family: "Times New Roman" mso-hansi-font-family: "Times New Roman" mso-bidi-font-family: Arial mso-bidi-theme-font: minor-bidi mso-ansi-font-size: 9.0pt">(Papanicolaou method<span lang="AR-SA" style="font-family: "Arial","sans-serif" font-size: 10pt mso-ascii-font-family: "Times New Roman" mso-hansi-font-family: "Times New Roman" mso-bidi-font-family: Arial mso-bidi-theme-font: minor-bidi mso-ansi-font-size: 9.0pt">) رنگ آمیزی گردید. تغییرات کمی و کیفی اسمیرسیتولوژی (Cytological smear) در هر اسلاید بررسی شد. ارزیابی کمی با نرم افزار Motic plus2 و 50 سلول در هر اسلاید صورت گرفت. افراد در گروه‌ها از نظر سن و جنس یکسان سازی شدند.<span lang="AR-SA" style="font-family: "B Mitra" font-size: 12pt mso-ascii-font-family: Cambria mso-ascii-theme-font: major-latin mso-fareast-font-family: "Times New Roman" mso-hansi-font-family: Cambria mso-hansi-theme-font: major-latin"><o:p /> یافته‌ها: افزایش اندازه هسته، نسبت اندازه هسته به سیتوپلاسم، Ferret ratio (نسبت قطر بزرگ به کوچک هسته) و کاهش اندازه سیتوپلاسم به ترتیب در نواحی زبان، مخاط باکال و کف دهان افراد سیگاری، قلیانی و سالم وجود داشت (001/0≥p). اختلاف آماری معنی داری از نظر درصد کاریورکسی Karyorrhexis))، هسته جند لوبه Multilobed nuclei)) و واکوئولیزاسیون<a href="http://www.google.com/url?sa=t&rct=j&q=vacuolozation&source=web&cd=1&cad=rja&ved=0CCwQFjAA&url=http%3A%2F%2Fen.wikipedia.org%2Fwiki%2FVacuolization&ei=-LjPUev3B4vntQaG6oHwBw&usg=AFQjCNHdqB5xcrIde475VTCaKHH1WeIeIg&bvm=bv.48572450,d.bGE">Vacuolization) </a>) سیتوپلاسم در مخاط دهان افراد سیگاری و قلیانی و سالم در نواحی مخاط باکال (8/0 p=)، زبان و کف دهان (9/0 =p) مشاهده نشد، اما درصد التهاب (001/0 >p) و کاندیدا (002/0 =p) در افراد سیگاری و قلیانی بیشتر ازسالم بود.<span lang="AR-SA" style="font-family: "B Mitra" font-size: 12pt mso-ascii-font-family: Cambria mso-ascii-theme-font: major-latin mso-fareast-font-family: "Times New Roman" mso-hansi-font-family: Cambria mso-hansi-theme-font: major-latin"><o:p /> نتیجه‌گیری: سیگار و قلیان در ایجاد تغییرات کمی سیتومورفومتری (Cytomorphometry) مخاط دهان مؤثرند، اما تأثیر سیگار در این زمینه بیشتر از قلیان است.<o:p /> Background: Oral mucosa of cigarette and hookah users is more susceptible for malignant changes, so the purpose of this study is to assess the impact of cigarette and hookah on morphometry features of oral mucosa.<o:p /> Methods: In this Historical Cohort study, Cytological smears prepared from three regions of oral mucosa (Buccal mucosa, lateral border of tongue and floor of the mouth) in 40 smokers, 40 hookah users , 40 non-smoker and non-hooka users were stained by Papanicolaou method. A quantitative and qualitative change in cytology smears was evaluated in each slide. Quantitative evaluation of 50 cells per slide was performed with Motic Plus2 software. Practitioners were matched with age and sex.<o:p /> Results: Increased nuclear size, nuclear to cytoplasmic ratio, Ferret ratio, reduced size of cytoplasm was observed in cigarette users, hookah users, non-smokers and non hookah users respectively (p<0.001). No Significant difference in karyorrhexis, multilobed nuclei and cytoplasmic vacuolization in buccal mucosa,tongue and floor of the mouth of cigarrete, hookan users(p=0.8) and non smoker-non hookan users was seen. (p3=0.9) but inflammation(p< 0/001) and candidiasis (p=0/002)were more in smokersand hookah users than non smoker and non hookah users respectively. <o:p /> Conclusions: Cigarette and hookah are effective in cyto-morphometric changes of oral mucosa, but in this area, The effect of cigarette is more than hookah.<o:p /> سیگار، قلیان، سیتومورفومتری، سیتولوژی، مخاط دهان.
