fa خصوصیات بالینی و یافته‌های آزمایشگاهی ٢٧٤ بیمار دارای کهیر مزمن و مرور مقالات Clinical characteristics and laboratory findings of 274 patients with chronic urticarial and review of literatures ایمونولوژی Immunology پژوهشي Research زمینه و هدف: کهیر از بیماری‌های شایع پوستی با شیوع 0.3 تا 11.3 درصد است. 20 درصد از مردم در طول زندگی به آن مبتلا می‌گردند. کهیر در غالب موارد علت شناخته شده ای ندارد. هدف از این مطالعه توصیف علائم بالینی، تست‌های آزمایشگاهی و مشخصات فردی تعداد 274 بیمار دارای کهیر مزمن از سال 1385 تا 1390 می‌باشد.<o:p /> روش کار: این تحقیق یک مطالعه توصیفی - تحلیلی آینده نگر می باشد که در مدت 4سال و نیم (مهر 86 تا اسفند 90) در کلینیک‌های آلرژی بیمارستان بوعلی سینا و کلینیک طوبی در ساری بر روی 274 بیمار (195 مونث و 79مذکر) دارای کهیر مزمن با سن بین 2 تا 75 سال داشته اند، انجام شده است. کلیه بیماران مورد بررسی آزمایشات مختلف خون شامل CBC, diff, ESR, T4, TSH, TPO, TGA, IgE, AST, ALT, BUN, Cr, ANA, CH50, C3, C4, S/E, U/A به همراه آنتی بادی‌های میکروب هلیکوباکتر پیلوری (Helicobacter pylori) قرار گرفتند.<o:p /> یافته ها: در این مطالعه کهیر مزمن در سن‌های بین 20 تا 40 سال شایع‌تر بوده است (55%). 45 نفر کهیر فیزیکی داشته‌اند (42/16درصد) که 21مورد با درموگرافیسم یا نوشتن روی پوست (Dermographism) شایع تر از بقیه بوده است. آنژیو ادم (Angioedema) در 144 مورد از بیماران دیده شده است (5/52درصد). سطح IgE توتال سرم در 46 درصد از بیماران افزایش داشته است (128 نفر). سابقه بیماری های آلرژیک دیگر در 128 نفر (46%) مثبت می‌باشد. عمکرد تیروئید در 4 مورد غیر عادی بوده است و آنتی بادی‌های ضد تیروئید در 50 مورد (24/18 درصد) افزایش داشته است. افزایش آنتی‌بادی‌‌های ضد هلیکوباکتر پیلوری در 23/69 درصد وجود داشته که 90 درصد آنها بالای 18 سال داشته‌اند.تست‌های ANA, CH50, C3, C4 و آزمایش مدفوع در تمام بیماران ما نرمال بوده است.<o:p /> نتیجه‌گیری: در کهیر مزمن اغلب آزمایشات معمول شامل CBC، عملکرد کلیه و کبد نرمال هستند و انجام آنها توصیه نمی‌شوند.<o:p /> Background: Chronic urticaria is a common skin disorder with incidence of 0.3 to 11.3 percent people. Up to 20 % of general population affected it in their life span. There is not known etiology in the most cases of chronic urticarial. The aim of this study was to detect clinical manifestation, laboratory tests and demographic of 274 patients with chronic urticarial in Sari, Iran.<o:p /> Methods: In this descriptive-cross sectional study, we evaluated 274 patients (195 female and 79 male) with chronic urticaria who were referred to allergic centers (Booali and Tooba clinics) in Sari ,for 4 and half years (Dec 2008tillMarch 2012). All of them had ages between 2 to 75 years. We evaluate demographic characteristics, different laboratory tests such as CBC, ESR, CH50, C3, C4, IgE, and anti-thyroid and helicobacter pylori antibodies in them.<o:p /> Results: Chronic urticaria is more common from 20 to 40 years of age in both genders (55%). Physical urticaria was found in 42 patients, with more common cases of dermographism (n=21). Angioedema was seen in 144 patients (52.5%). Total IgE increased in 128 of cases (46%). History of allergic disorders were positive in 128 cases (46%).Anti thyroid antibodies increased in 50 cases (18.24%) with 4 patients with abnormal thyroid function tests. Helicobacter Pylori antibodies, IgG and IgA were increased in 69.23% whom 90% were above 18 years old. Antinuclear antibody, CH50, C3, C4, Stool exam were normal in all of them.<o:p /> Conclusions: In Chronic urticaria often laboratory tests including CBC, liver and kidney functions are normal and better not to be recommended.<o:p /> کهیر، تست‌های آزمایشگاهی، آنتی بادی تیروئید.
