fa بررسی ارتباط بین عوامل روانشناختی و ناتوانی جنسی با کیفیت زندگی بیماران مرد پس از جراحی بای پاس عروق کرونر Assessment of the relationship between psychological factors and impotency with quality of life of male patients following CABG روانشناسی بالینی Clinical Psychiatry پژوهشي Research زمینه و هدف: با توجه به افزایش شیوع جراحی بای پاس عروق کرونر (Coronary Artery Bypass Graft Surgery) و نقش برجسته عوامل روانشناختی و ناتوانی جنسی در پیش آگهی و عود این بیماری، مطالعه حاضر با هدف تعیین رابطه بین عوامل روانشناختی و ناتوانی جنسی با کیفیت زندگی بیماران مرد پس از جراحی بای پاس عروق کرونر در سال 1390 انجام شده است.<o:p /> روش کار: در این مطالعه توصیفی- تحلیلی 160 بیمار مرد، 4 هفته پس از جراحی بای پاس عروق کرونر به صورت نمونه گیری تصادفی انتخاب شدند و پرسشنامه های مربوط به اطلاعات فردی، پرسشنامه سنجش اضطراب، افسردگی و استرس (Depression Anxiety Stress Scales)، سنجش ناتوانی جنسی (<a href="http://www.google.com/url?sa=t&rct=j&q=IIEF-5&source=web&cd=1&cad=rja&ved=0CDAQFjAA&url=http%3A%2F%2Fwww.hiv.va.gov%2Fprovider%2Fmanual-primary-care%2Furology-tool2.asp&ei=UOXaUNfAC86N0QGatIGgBQ&usg=AFQjCNFZ45Z-t3jNRdny84H3nJZxsoQW4A&bvm=bv.1355534169,d.dmQ"><span style="color: windowtext font-family: "Times New Roman","serif" font-size: 9pt text-decoration: none mso-bidi-language: FA mso-bidi-font-size: 10.0pt mso-bidi-font-family: "B Mitra" text-underline: none" dir="ltr">The International Index of Erectile Function (IIEF-5) Questionnaire </a>) و کیفیت زندگی (The Short Form (36) Health Survey ) را تکمیل کردند. داده ها توسط نرم افزار <st1:stockticker w:st="on">SPSS</st1:stockticker> و آزمون تی مستقل و همبستگی پیرسون مورد تجزیه و تحلیل قرار گرفت.<o:p /> یافتهها: میانگین سنی افراد مورد مطالعه 60 سال بود. بیماران کمترین نمره را در ابعاد محدودیت نقش جسمانی و هیجانی و بیشترین نمره را در ابعاد سلامت روان و سلامت عمومی کسب کرده بودند. از بین متغیرهای روانشناسی مورد بحث، میانگین نمره افسردگی رابطه معنی داری با میانگین نمره کیفیت زندگی داشت (02/0=p). 4/62 درصد بیماران دارای درجاتی از افسردگی، 7/86 درصد دارای درجاتی از استرس و 3/47 درصد دارای درجاتی از اضطراب بودند. ضمنا 5/85 درصد بیماران دارای ناتوانی جنسی از خفیف تا شدید بودند. بین میانگین نمره ناتوانی جنسی با کیفیت زندگی ارتباط معنی داری نبود. اما بین نمره اضطراب (03/0=p) و افسردگی (001/0p=) با ناتوانی جنسی رابطه معنی داری وجود داشت. همچنین بیماران دیابتی نمره بالاتری از افسردگی و استرس را نسبت به غیر دیابتی ها گزارش کردند.<o:p /> نتیجهگیری: از آنجا که افسردگی باعث کیفیت زندگی نامطلوب در بیماران پس از جراحی بای پاس عروق کرونر می گردد، لازم است با انجام مداخلاتی، درمان افسردگی این بیماران، در دستور کار متخصصان قرار گیرد.<o:p /> Background: Given the increase in prevalence of Coronary Artery Bypass Graft (CABG) surgery and the significant role of psychological factors in prognosis and relapse of cardiac disease, the present study is designed to determine the relationship between psychological factors or impotency and quality of life of male patients following CABG.<o:p /> Methods: In this descriptive- analytical study 160 male patients were randomly selected four weeks after heart surgery and were administered DASS-21, IIEF-5 and SF-36 to assess anxiety, depression and stress, impotency and quality of life, respectively. Regression analyses (One-way ANOVA) and Chi-square were performed to analyze the data.<o:p /> Results: Findings showed that the average age of patients was 60 (±10) years. Among psychological variables the average score for depression was significantly related to the average scores of quality of life (p=0.02). Furthermore, 62.4% of patients had some degrees of depression 86.7% had some degrees of stress and 47.3% had anxiety to some degree. Also, 85.5% of patients displayed mild to severe levels of erectile dysfunction which were significantly correlated with scores in anxiety (p=0.03) and depression (p=0.001). Finally, diabetic patients had higher scores in depression and stress compared to non diabetic patients.<o:p /> Conclusion: The findings are discussed in the context of proposing interventional strategies for improving psychological conditions of CABG patients.<o:p /> جراحی بای پاس عروق کرونر، ناتوانی جنسی، اضطراب، افسردگی، استرس.
