میزان اتوآنتیبادی ضد گلوتامیک اسید در بیماران دیابتی نوع دو و وابستگان درجه اول آنها
ANTI-GAD AUTOANTIBODY LEVELS IN TYPE II DIABETES PATIENTS AND THEIR FIRST DEGREE RELATIVES

<p dir="rtl">آنزیم گلوتامیک اسید دکربوکسیلاز( GAD ) که مسئول تبدیل گلوتامیک اسید به گاما آمینوبوتیریک اسید است یک نوروترانسمیتر مهاری غالب در سیستم اعصاب مرکزی میباشد. پروتئین GAD ، 2 ایزومر مولکولی دارد که وزن مولکولی یکی از آنها 65 کیلودالتون ( GAD65 ) و دیگری 67 کیلودالتون( GAD67 ) است. ژن 65 GAD روی کروموزوم 10 قرار دارد و در سلولهای بتای پانکراس بیان میشود. در بیماران IDDM غلظتهای بالای GADA مشاهده میشود و 65 GAD نقش مهمی در پیشگویی دیابت IDDM ایفا میکند. سنتز این اتو آنتیژن در سلولهای بتا توسط غلظتهای زیاد گلوکز تحریک شده و افزایش مییابد، بنابراین افزایش عملکرد سلولهای جزایر بتای پانکراس سبب افزایش بیان ژن و سنتز پروتئین 65 GAD در این سلولها شده و منجر به تولید آنتیبادی بر ضد آن میگردد( Anti-GAD ). در برخی از بیماران مبتلا به دیابت نوع دو میزان 65 Anti-GAD افزایش یافته ( Anti-GAD مثبت) و بیمار بعد از مدت کوتاهی به انسولین وابسته میشود. در این مطالعه میزان آنتیبادی ضد گلوتامیک اسید دکربوکسیلاز(65 Anti-GAD ) در 50 نفر از بیماران دیابتی نوع دو ایرانی و 32 نفر از وابستگان درجه اول آنها و 56 نفر از افراد سالم تعیین و مقایسه گردید. تعیین میزان Anti-GAD با روش ELISA صورت گرفت. میانگین میزان Anti-GAD در افراد سالم 01/7 ± 64/6 نانوگرم در میلیلیتر و در افراد بیمار 59/13 ± 76/10 نانوگرم در میلیلیتر و در وابستگان درجه اول آنها 1/32 ± 71/19 نانوگرم در میلیلیتر به دست آمد. با در نظر گرفتن Cut off ، درصد فراوانی Anti-GAD مثبت در بیماران 18% و در وابستگان درجه اول آنها 9/21% بود و هیچ یک از افراد طبیعی، تیتر مثبت 65 Anti-GAD را نداشتند. از نظر فراوانی، میزان Anti-GAD مثبت در بیماران نسبت به افراد سالم افزایش معنیداری را نشان داد(001/0 P< ) که این حالت در وابستگان درجه اول بیماران نسبت به افراد سالم نیز وجود داشت(001/0= P ). براساس نتایج حاصل از این مطالعه به نظر میرسد که تخمین میزان Anti-GAD میتواند به عنوان یک ابزار مفید در پیشگویی ابتلا به دیابت و وابستگی آن به انسولین به کار گرفته شود. </p><p dir="rtl" >
Glutamic Acid Decarboxylase(GAD) catalyses the conversion of glutamic acid to Gama amino Butyric Acid(GABA) which is one of the major inhibitory neurotransmitters in central nervous system. GAD has two isoforms with molecular weights of 65 Kda(GAD 65) and 67 Kda (GAD 67). GAD 65 gene is located on chromosome 10 and expressed in β-cells of pancrease. The presence of high concentrations of Anti-GAD Antibodies(GADA) in serum of patients with IDDM is evident. Evaluation of GADA levels in individuals with NIDDM and their first degree relatives can also be used as a predictor of late insuline dependency. High concentrations of glucose induce the production of GAD 65 in β-cells which is associated with an increase in GAD 65 gene expression. It has been shown that patients with NIDDM may produce higher levels of GADA and after short period of time developing IDDM. In the present study anti-GAD levels were determined and compared in 50 type II Iranian diabetic patients, in 32 of their first degree relatives and in 56 healthy subjects. The evaluation of GADA levels were carried out by the use of ELISA method. The mean level of anti-GAD in the healthy subjects was 6.64±7.01ng/ml, in the patients was 10.76±13.59ng/ml and in their first degree relatives was 19.71±32.1ng/ml. In regard to the cut off, 18% of patients had seropositive anti-GAD and 21.9% of their first degree relatives had seropositive anti-GAD but all of the healthy subjects had seronegative anti-GAD. The frequencies of anti-GAD positivity in patients and their first degree relatives compared with that of healthy subjects were significantly higher(P<0.001 and P=0.001 respectively). It can be concluded that the determination of anti-GAD in healthy subjects and their first degree relatives of type II diabetic patients can be used to predict the development of diabetes and insuline dependency.
