به کارگیری مدل اثرات تصادفی در تعیین عوامل موثر بر اختلال رشد کودکان زیر 2سال مراجعه کننده به مراکز بهداشت شرق تهران

<p> <strong> زمینه و هدف: </strong>اختلال رشد ( Failure To Thrive-FTT )، تاخیر یا توقف در رشد کودک است و عدم رسیدگی به این اختلال باعث بروز عوارض جدیتری همچون مرگ و میر، افزایش سایر بیماری های مرتبط، کاهش یادگیری و ناتوانی و معلولیت ذهنی، عاطفی یا جسمی می شود. بنابراین با توجه به عوارض جدی اختلال رشد در آینده کودکان در این مطالعه به بررسی عوامل موثر در بروز اختلال رشد کودکان پرداخته می شود. </p><p> <strong> روش کار: </strong>جامعه مورد بررسی کودکان زیر 2 سال مراجعه کننده به مراکز بهداشت شرق تهران بودند که تعداد 1660 کودک با استفاده از روش نمونهگیری خوشهای دومرحلهای وبا توجه به معیارهای ورود که عبارتند از عدم بیماریهای خاص هنگام تولد و مراجعه منظم به مراکز درمانی، به عنوان نمونه انتخاب شدند. پس از جمع آوری اطلاعات دو سال اول زندگی از پروندههای این کودکان، عوامل موثر بر اختلال رشد با استفاده از نرم افزار SAS ومدل رگرسیون با اثرات تصادفی مورد تجزیه و تحلیل قرار گرفت. </p><p> <strong> یافته ها: </strong>میانگین ( ± انحراف معیار) سن مادران در زمان تولد برابر46/5 ± 3/27بود، 86% (1428نفر) از مادران آن ها خانه دار بودند و 46% (764نفر) آن ها تحصیلاتی پایین تر از سطح دیپلم داشتند ولی رابطه معناداری بین میزان تحصیلات مادر و اختلال رشد وجود نداشت (290/0 p= ). میانگین( ± انحراف معیار) وزن هنگام تولد کودکان 03/463 ± 3202 گرم بود که 1/5% (85نفر) آنها کم وزن (کمتر از 2500 گرم) به دنیا آمده بودند و 1/68% (1130نفر) از کودکان حداقل در یک دوره زمانی دچار اختلال رشد شده بودند. ابتلا به اسهال، عفونت ادراری و تنفسی، دندان درآوردن، تغذیه، قطع شیر مادر و سایر بیماریها تاثیر معناداری در ایجاد اختلال رشد کودک داشتند (05/0> p ) که از میان آنها بزرگترین مقدار نسبت بختها مربوط به عامل ابتلا به اسهال بود، واریانس جزء تصادفی مدل نیز معنادار شد (024/0= p ) که نشان دهنده موثر بودن ویژگیهای فردی کودکان در اختلال رشد علاوه بر متغیر های شناخته شده است. </p><strong>نتیجهگیری: </strong>با توجه به عوارض فراوان و جدی این اختلال در آینده کودک و بالا بودن میزان شیوع اختلال رشد در کودکان زیر 2 سال، لزوم رسیدگی به این اختلال کاملاً احساس می شود. با توجه به معناداری اثر تصادفی، عوامل شناخته شده به تنهایی برای پیش بینی اختلال رشد کودکان کافی نبوده و ویژگیهای فردی کودکان و عوامل ژنتیکی و محیطی نیز در بروز اختلال رشد موثر است.
<p> <strong> Background </strong>: Failure To Thrive (FTT) is the delay or suspension in child growth not dealing with this disorder may lead to more serious consequences like increased death rate, appearance of other relevant diseases, reduced learning, and mental, emotional, or physical disabilities. Given the serious symptoms of failure to thrive in children’s future, this paper tries to identify and investigate factors affected in FTT. </p><p> <strong> Methods: </strong>The study population included children below 2 years old visiting the Health Center of East Tehran from which 1660 infants using two-stage cluster sampling method were selected according to the input criteria (not suffering from any particular disease at birth, and having regular visit to health centers). After gathering data from the first two years of the infants’ lives, factors associated with FTT using SAS software and Random Effect Regression Model were analyzed. </p><p> <strong> Results: </strong>Mean age (± SD) of mothers at birth time was 27.3 ± 5.46 years and 46% (764) of the mothers had an education lower than high school diploma but there was no significant relationship between mothers’ education and FTT (p = 0.290). Mean weight (± SD) of children at birth was 3202 ± 463.03 g and 5.1% (85) of them were born with underweight (2500g) and 68.1% (1130) of the children were affected by FTT at least in one period. Factors associated with diarrhea, infection, teething, nourishment, weaning, and other diseases had significant role in causing FTT (p < 0.05), out of which the cause of diarrhea was the most likely factor. In addition, the model’s random section became significant (p < 0.05). </p><p> <strong> Conclusion: </strong>Given the abundance and graveness of the symptoms of this disorder in child’s future and the high likelihood of occurrence of this disorder among infants below 2 years, a strong need is felt to deal with this impairment. Given the significance of the random effect, the known causes alone are not enough for prediction of FTT among infants and genetic and environmental factors play a role in occurrence of FTT as well. </p>
