fa بررسی اثرات تغذیه با شیرمادر و هم اتاقی مادر و نوزاد بر میزان عفونت نوزادی در مرکز آموزشی درمانی فیروزگر - تهران Evaluation of Breast-Feeding and Roomong-In Effects on the Risk of Neonatal Infections بیماری‌های اطفال Pediatric Disease پژوهشي Research بر میزان شیوع (rooming-in-ri) برای تعیین اثرات تغذیه با شیر مادر واجرای طرح هم اتاقی مادر ونوزاد عفونتهای دوره نوزادی مطالعه حاضر به صورت مورد شاهد در مرکز اموزشی درمانی فیروزگر انجام شده است.این بررسی بر روی پرونده های بالینی نوزادان در طی 3 دوره انجام شده: 1-دوره اول ایمنی دوره قبل از اجرای طرح ،که نوزادان در نرسی نگهداری می شدند(سالهای 64-1362) 2-دوره دوم یا دوره انتقالی که نوزادان روزها در نرسری نگهداری و فقط شبها در کنار مادرشان بودند. (سالهای 69-1364) و3-دوره سوم یعنی دوره اجرای کامل طرح(71-1369)در این بررسی ابتدا تمامی پرونده های نوزادان متولد شده در دوره سوم (2106پرونده)به عنوان نمونه مشخص وبه تعداد تقریبا مساوی از پرونده های نوزادان متولد شده در دورهای اول ودوم به ترتیب بعنوان شاهد اول و دوم بطور کاملا تصادفی انتخاب وبررسی شده است.در این 3دورهمیزان تشابه نوزادان،ابنلای انها به عفونتها و تب با یکدیگر مقایسه شده است. درصد نوزادان پره ترم ونوزادان متولد شده به طریقه سزارین در دوره سوم بطور معنی داری بیش از دوره قبل بوده است.در مجموع درصد ابتلا به عفونت در دوره اول 3/2درصد،در دوره دوم 17/2 درصد ودر دوره سوم 5/1 درصد بوده است.درصدابتلا به تب دردوره اول 6/0درصد دردوره دوم 57/0 درصد ودر دوره سوم 04/0 درصد (تنها یک نفر)بوده است.(خطر ابتلا به تب در دوره سوم 12 بار کاهش داشته است).هیچکدام از نوزادان در دوره سوم به اسهال مبتلا نشده است.خطر بروز سپتی سمی ،راش،بیماریهای حاد تنفسی و عفونت بند ناف در کل سه دوره اختلاف معنی داری نداشته است. بطور کلی در این بررسی،در نوزادان طبیعی(یعنی وزن زمان تولد بالای 4500 گرم فول ترم وبا اپگار بالای 8در دقیقه پنجم)خطر بروز عفونتها در مجموع در دوره سوم 44درصد کاهش یافته وخطر بروز تب،اسهال ویپتی سمی به صفر رسیده است. <div style="mso-element: frame mso-element-frame-width: 527.5pt mso-element-frame-height: 253.4pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt"><tbody><tr><td style="BORDER-BOTTOM: #f0f0f0 BORDER-LEFT: #f0f0f0 PADDING-BOTTOM: 0in BACKGROUND-COLOR: transparent PADDING-LEFT: 0in PADDING-RIGHT: 0in BORDER-TOP: #f0f0f0 BORDER-RIGHT: #f0f0f0 PADDING-TOP: 0in" height="338" valign="top" align="left"><p style="LINE-HEIGHT: 17.5pt MARGIN: 0in 0in 0pt 15.35pt mso-element: frame mso-element-frame-width: 527.5pt mso-element-frame-height: 253.4pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">In order to evaluate the effect of breast feeding and rooming-in (R.I.) on the incidence of neonatal infections <p style="LINE-HEIGHT: 1.2pt MARGIN: 0in 0in 0pt 524.6pt mso-element: frame mso-element-frame-width: 527.5pt mso-element-frame-height: 253.4pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">, <p style="LINE-HEIGHT: 18.95pt MARGIN: 0in 0in 0pt 0.7pt mso-element: frame mso-element-frame-width: 527.5pt mso-element-frame-height: 253.4pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">this study was performed in Firoozgar Hospital - Tehran from 1982 through 7992. <p style="TEXT-JUSTIFY: inter-ideograph TEXT-ALIGN: justify LINE-HEIGHT: 20.6pt TEXT-INDENT: 15.35pt MARGIN: 0in 0.95pt 0pt 0in mso-element: frame mso-element-frame-width: 527.5pt mso-element-frame-height: 253.4pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">This study was designed in order to review the birth records of neonates in three periods: 1, Pre-rooming- in (1982-1985),2, Partialy rooming-in (1985-1990) and 3 rooming-in period (7990-1992).
