fa گزارش 200 مورد مبتلا به زونا REPORT OF 200 ZONA PATIENTS پوست Dermatology موردنگاري case report این مطالعه برای تعیین سن، جنس، محل درگیری درماتومی و سمت ابتلا در مبتلایان به زونا انجام شد. به این منظور از بین بیماران درمانگاه پوست بیمارستان فیروز آبادی دانشگاه علوم پزشکی ایران در تهران کلیه بیمارانی که برای نخستین بار به ضایعات کیسکی تاولی (vesiculobullous) گروهی دردناک در زمینه اریتمائی (erythematous) و با توزیع درماتومی مبتلا شده بودند ( به شرط عدم وجود عوامل مستعد کننده و تطابق سیر بالینی ابتلا به بیماری حداقل به مدت 2 هفته پس از سه معاینه با فاصله زمانی 7 روز از اولین مراجعه با تشخیص زونا، انتخاب شده مورد مطالعه قرار گرفتند. در طول مدت مطالعه ( از ابتدای شهریور 1375 لغایت پایان آذر 1377) مجموعا 200 مورد زونا مشاهده شد (18/1% از مراجعه کننگان به درمانگاه پوست) دامنه سنی ابتلا از 9 تا 90 سال و میانگین سنی مبتلایان ( میانگین خطای استاندارد) برابر 89/41 37/1 سال بود. 65% از مبتلایان سنی برابر یا کمتراز 50 سال و 35% سنی کمتر از 30 سال داشتند که نشان دهنده پائینتر بودن قابل توجه سن ابتلا در مقایسه با آمار ارائه شده در سایر مطالعات می باشد. اختلاف معنی داری بین میانگین سنی مبتلایان زن و مرد مشاهده نشد. شایعترین درماتوم های درگیر این مطالعه به ترتیب نواحی سینه ای (33%) کمری- خاجی (5/030%) گردنی (20%) و سه قلوئی (16%) بود و یک مورد (5/0%) زونای منتشر مشاهده شد. در این مطالعه درماتوم های سینه ای شایعترین محل درگیری بودند اما فراوانی درگیری آنها در مقایسه با آمار ارائه شده در سایر مطالعات (53 تا 55%) کمتر بوده است. فراوانی درگیری ناحیه کمری خاجی از فراوانی ذکر شده در سایر مطالعات (11%) بیشتر بوده است میانگین سن درگیری درماتوم سه قلوئی از میانگین سن درگیری درماتوم های سینه ای به طور معنی داری بیشتر بوده است اختلاف دو میانگین= 98/12 سال و011/0= p از نظر محل درگیری و سمت درگیر راست یا چپ بین دو جنس و نیز در سنین مختلف، اختلاف آماری معنی داری مشاهده نشد. تفاوت معنی داری بین درماتوم های درگیر در طرف راست و چپ وجود نداشت. <div style="mso-element: frame mso-element-frame-width: 517.2pt mso-element-frame-height: 416.85pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt"><table cellspacing="0" cellpadding="0" width="689" height="556" vspace="0" hspace="0"><tbody><tr><td style="BORDER-BOTTOM: #f0f0f0 BORDER-LEFT: #f0f0f0 PADDING-BOTTOM: 0in BACKGROUND-COLOR: transparent PADDING-LEFT: 0in PADDING-RIGHT: 0in BORDER-TOP: #f0f0f0 BORDER-RIGHT: #f0f0f0 PADDING-TOP: 0in" height="556" valign="top" align="left"><p style="LINE-HEIGHT: 19.9pt MARGIN: 0in 0in 0pt 0.25in mso-element: frame mso-element-frame-width: 517.2pt mso-element-frame-height: 416.85pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">This study was performed to determine age, sex, site and side of involvement in patients with herpes zoster. <p style="TEXT-JUSTIFY: inter-ideograph TEXT-ALIGN: justify LINE-HEIGHT: 19.9pt MARGIN: 0in 0in 0pt 1.65pt mso-element: frame mso-element-frame-width: 517.2pt mso-element-frame-height: 416.85pt mso-element-wrap: auto mso-element-anchor-horizontal: margin mso-element-left: .05pt mso-element-top: .05pt mso-line-height-rule: exactly" class="Style">For this purpose, all patients of dermatology clinic of Firuzabadi Hospital in Thehran who presented with painful grouped vesiculobullous lesions with a dermatomal distribution on an erythematous base for the first time, in whom no predisposing factor could be detected, and had a clinical course compatible with herpes zoster (at least of two weeks duration) were considered as herpes zoster cases after three follow up visits with weekly intervals from the first visit and were included in this study. 200 cases of herpes zoster were found (1.18% of dermatology clinic cases) from August 22, 1996 through December 21, 1998. The ages of patients ranged from 9 to 90 years and the mean age of involvement (mean ± Sf) was 41.89 ± 1.37 years. 65% of the patients were 50 years old or younger and 35% were younger than 30 years. This shows that in this study the age of involvement is significantly lower than other series. There was no significant difference between mean ages of males and females. In this study, the most common involved dermatomes were thoracic, but the frequencies of involvement of these dermatomes were lower than what reported in other series (53-55%). The frequency of involvement of lumbosacral dermatome was higher than other series (11 %). The mean age of trigeminal involvement was significantly higher than the mean age of the patients with thoracic involvement (difference of two means = 12.98 years and P = 0.011). There were no significant differences between the sites and the sides (right or left) of involvement in males and females as well as between different age groups.
