ارزیابی تشخیصی و درمانی بیماران با آسیب اورولوژیک مراجعه کننده به اورژانس بیمارستان حضرت رسول اکرم (ص) طی سال های 87-1386
Assessment of the diagnostic and therapeutic characteristics of patients with urologic trauma referred to Hazrat Rasoul-e-Akram Hospital between 2007 and 2008

<p style="DIRECTION: rtl"> <strong> زمینه و هدف: </strong>ایران از نظر مرگ و میر ناشی از حوادث رانندگی رتبه بالایی در سطح جهان دارد. بسیاری از این بیماران دارای آسیب اورولوژیک نیز می باشند. میزان آسیب اورولوژیک بیماران با ترومای متعدد در کشور ما 12-10 درصد میباشد. هدف از این مطالعه ارزیابی اپیدمیولوژیک تعدادی از بیماران با ترومای اورولوژیک و ارزیابی وضعیت تشخیصی و درمانی آنها می باشد. </p><p> <strong> روش کار: </strong>این مطالعه توصیفی- مقطعی بوده است. اطلاعات شامل عامل تروما، ضایعات اورولوژیک، ضایعات همراه، علائم حیاتی، روش تشخیص، نوع درمان و پیامد نهائی از پرونده 76 بیمار مبتلا به ترومای متعدد، که دارای ترومای اورولوژیک نیز بوده اند و به اورژانس بیمارستان حضرت رسول اکرم طی سال های 87-1386 مراجعه داشته اند، جمع آوری و مورد ارزیابی قرار گرفت. در آنالیز آماری از تستهای آماری t ، Chi-square ، One way ANOVA و معادل های غیرپارامتریک آن ها استفاده شد. از SPSS V.16.0 نیز استفاده شد. </p><p> <strong> یافتهها: </strong>میانگین سنی بیماران مراجعه کننده 2 ± 33 سال بوده است. میانگین زمان بستری 6 ± 6/11 روز بود. 35 نفر (5/44%) آسیب کلیوی در گرید I تا V داشتند. 25 نفر (3/26 %) آسیب مثانه، 14 نفر (4/18 %) آسیب مجرا، 37 نفر (7/48 %) آسیب همراه، 13 نفر (1/17 %) آسیب دستگاه ژنیتال خارجی و 4 نفر (2/5 %) آسیب حالب داشتند. شایع ترین آسیب همراه، شکستگی لگن در 19 نفر (7/24) %) بوده است. از کل بیماران 3 مورد فوت شدند؛ افراد فوت شده همراه با آسیب های غیر اورولوژیک بودند. 43 مورد (6/55%) بهبودی کامل و 30 نفر (5/39%) احتیاج به جراحی تاخیری داشته اند. </p><p> <strong> نتیجهگیری: </strong>عوارض بعد از جراحی اورولوژیک کم و هیچ بیماری به علت آسیب اورولوژیک فوت نشده است. بین عامل تروما و آسیب ایجاد شده ارتباط معنی داری وجود نداشته که می تواند ناشی از تعداد کم بیماران، نقص در پرونده و یا فوت بیماران با آسیب شدید در صحنه تصادف پیش از رسیدن به مرکز اورژانس باشد. عوارض حین درمان به خوبی کنترل شده وامکانات تشخیصی و درمانی مورد نیاز بیماران کافی بوده است. </p>
<p> <strong> Introduction </strong>: The mortality rate of car crashes in Iran is very high worldwide. Most of these injured persons have urologic injuries. In our country, the prevalence of urologic injuries accompanied by multiple traumas is about 10-12%. The aim of this study was to evaluate the epidemiology of patients with urologic trauma and the diagnostic and therapeutic modalities. </p><p> <strong> Methods </strong>: We used a check list that consisted of the etiology of trauma, urologic trauma, co-morbid trauma, vital signs, diagnostic procedures, type of treatment and final outcome. The files of 76 patients with multiple injuries accompanied by urologic trauma that had referred to emergency department of Rasool Akram hospital between 2007 and 2008, were evaluated. We used t, Chi-square, One way ANOVA and their equivalent non parametric tests in our statistical analysis. SPSS V. 16.0 was also used. </p><p> <strong> Results </strong>: Mean age of patients was 33±2 years. The mean hospitalization period was 11.6±6 days. There were 35 kidney injuries (44.5%) with grades I-V. Twenty-five patients (26.3%) had bladder injuries, 14(18.4%) urethral injuries, 37(48.7%) co-morbid injuries, 13(17.1%) external genitalia injuries (scrotal/testis) and 4(5.2%) had ureteral injuries. Pelvic fracture (19 patients- 24.7%) was the most common co-morbid injury. Overall there were three deaths that were accompanied by non-urologic co-morbidities. There were 43 patient (55.6%) with complete improvement and 30 patients (39.5%) who needed delayed surgery. </p><p> <strong> Conclusion </strong>: Complications of urologic surgery were low and nobody died of these complications. There was no significant relationship between trauma etiology and the severity of injury. This difference might have been due to less number of patients, incomplete hospital files or death of patient with severe injury at the time of accident. Complications during treatment were fully controlled. Our facilities for diagnosis and treatment meet our needs. </p>
