fa بررسی شیوع سرطان سلول‌های سنگفرشی سر و گردن و ارتباط آن با عوامل خطر در بالغین جوان Evaluation of Squamous Cell Carcinoma of the Head and Neck and Related Risk Factors in Young Adults آسیب‌شناسی Pathology پژوهشي Research <h2 style="MARGIN: 0cm 0cm 0pt DIRECTION: rtl" dir="rtl"></h2><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl"> زمینه وهدف:�سرطان‌های سلول‌های سنگفرشی�سر و�گردن (Head and Neck Squamous Cell Carcinoma-HNSCC) بیماری بالغین است, اما اخیراً گزارش هایی از افزایش بروز این بیماری در بالغین جوان دیده شده است. عوامل مختلفی در ایجاد این نوع سرطان دخیل می‌باشند. دو ریسک فاکتور مصرف سیگار و الکل, ریسک فاکتورهای اصلی�برای ابتلاء به HNSCC می‌باشند.�کم خونی(آنمی) مزمن که منجر به آتروفی مخاط می‌شود هم می‌تواند از عوامل افزایش استعداد به سرطان، به خصوص سرطان زبان باشد.� مطالعه موجود با هدف بررسی میزان شیوع�سرطان‌های سلول‌های سنگفرشی�سر و�گردن� و ارتباط این تومور با ریسک فاکتورهای آن در افراد 40 و زیر 40 سال (بیماران جوان)�در مقایسه باافراد بالای 40 سال انجام شد. </h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">روش کار: در این مطالعه مقطعی، توصیفی و گذشته نگر پرونده های 262 بیمار مبتلا به SCC سر و گردن طی 15 سال از سال 1371 تا 1386 در بیمارستان حضرت رسول اکرم(ص) تهران بررسی شد؛ 15 نفر�از�آن‌ها کمتر از 40 سال داشتند. توزیع جنسی، محل، ریسک فاکتورها، مرحله و تمایز تومور، درگیری لنفاوی، مصرف سیگار، مصرف الکل و وجود آنمی دراین بیماران مورد بررسی قرار گرفت�. داده‌ها در نرم افزار آماری SPSS V.15 وارد شده و توسط آزمون‌های آماری Chi-Square� و Mann-Whitney آنالیز می‌شود. 05/0p< معنی‌دار در نظر گرفته شد.</h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">یافته‌ها: ارتباط آماری معنی‌داری بین محل تومور با سن, جنس, مصرف سیگار, مصرف الکل�, آنمی و درگیری لنفاوی وجود دارد. بیشترین محل درگیر در هر دو جنس, لارنکس بود. درگیری لارنکس بیشتر در مردان(92%) و درگیری هیپوفارنکس بیشتر در زنان(61%) وجود داشت و�درگیری زبان به نسبت برابر در هر دو جنس وجود داشت. بیشترین محل‌های درگیر دربیماران جوان لارنکس(40%) و زبان(40%) و در گروه سنی بیشتر از 40 سال, لارنکس(5/82%) می‌باشد. در این مطالعه 8/5% بیماران 40 سال و کمتر از 40 سال سن داشتند که 6/66% از این بیماران فاقد ریسک فاکتور مصرف سیگار و 3/53% از این بیماران فاقد هر دو ریسک فاکتور مصرف سیگار و الکل بودند. </h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">نتیجه‌گیری:�با توجه به یافته‌های بالا و نبود ریسک فاکتورهای متداول در SCC سر و گردن در جوانان به نظر می‌آید که بیولوژی SCC سر و گردن در جوانان با افراد مسن‌تر متفاوت باشد و ممکن است ریسک فاکتورهای دیگری در پیشرفت SCC سر و گردن در بیماران جوان نقش داشته باشد.</h4> َBackground: Squamous Cell Carcinoma of the Head and Neck (HNSCC) is a disease of older adults, but recently there have been reports of a rising incidence in young adults. There are several etiological factors for HNSCC. The use of cigarette and alcohol are two main risk factors of tumors of the HNSCC. Chronic anemia which leads to mucosal atrophy may increase the susceptibility to carcinogenesis especially tongue cancer. The purpose of this research is to study the incidence of SCC of head and neck and relationship of this tumor to the risk factors in patients 40 years old or less in comparison to patient over 40 years old.Methods: In this retrospective, descriptive study medical files of 262 patients with SCC of the head and neck were studied (a 15 year period -from 1982 to 2007) in Hazrat Rasool-e- Akram hospital in <city w:st="on" >Tehran</city /></place />. Fifteen patients were young and aged less than 40 years old. This study compared sex, tumor site, risk factors, stage and grade of tumor, cervical node involvement and hematologic statues. All data were analyzed using Chi2 and Mann-Whitney tests. SPSS V.15 software was also used. Probability of less than 0.05% was considered as significant. Results: There was significant statically association between site of tumor and age, sex, consumption of cigarette, consumption of alcohol, anemia and cervical node involvement. Larynx was the most common site of involvement in both sexes. Laryngeal involvement was more common in men (92%) and involvement of hypopharynx was more in women (61%). Tongue involvement was the same in both sexes. The most common site of involvement in young patients was larynx (40%) and tongue (40%) and in patients older than 40 years it was larynx (82.5%). In this study 5.8% of patients were 40 years or less, 66.6% had never smoked and 53.3% had not shown any exposure to two major risk factors, alcohol and cigarette.Conclusion: Due to absence of two major risk factors in head and neck SCC of young patients it seems that biology of SCC of the head and neck in young and old adults could be different.
