fa مطالعه ارتباط سطح آنتی‌بادی‌های ضد ویروس اپشتاین بار در بیماران مبتلا به مولتیپل اسکلروز و تولید سایتوکاینهای IFN-γ، IL-4 وIL-12 توسط سلول‌های تک هسته‌ای خون محیطی فعال شده با میتوژن Evaluation of Serum Levels of Anti-Epstein Barr Virus Antibodies and Production of IFN-γ, IL-4 and IL-12 by Mitogen-activated Peripheral Blood Mononuclear Cells in Patients with Multiple Sclerosis بیماریهای عفونی Infectious Disease پژوهشي Research <h2 style="MARGIN: 0cm 0cm 0pt DIRECTION: rtl" dir="rtl"> زمینه و هدف:�مولتیپل اسکلروز (MS) شایعترین بیماری التهابی سیستم عصبی مرکزی می‌باشد که در آن طی یک روند خودایمنی، غلاف میلینی فیبرهای عصبی از بین می‌رود. در این بیماری لنفوسیت‌های CD4+ T بویژه زیر گروه TH1 در تخریب بافت عصبی نقش مهمی ایفا می‌کنند.�با توجه به نقش احتمالی ویروس اپشتاین بار در اتیولوژی و پاتوژنز بیماری MS، هدف از این مطالعه بررسی ارتباط سطح آنتی‌بادیهای ضد ویروس اپشتاین بار (EBV) با تولید سایتوکاین‌های TH1و TH2 می‌باشد. <h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">روش بررسی: در 68 بیمار مبتلا به مولتیپل اسکلروز در مراحل مختلف بیماری و 20 نفر از افراد سالم بعد�از�گرفتن نمونه خون وریدی سطح آنتی‌بادیهای ضد EBNA-1 و VCA از ویروس EBV به روش الایزا در پلاسما اندازه‌گیری گردید. سپس سلول‌های تک هسته‌ای خون محیطی (PBMC) به کمک فایکول جدا شد. PBMC به دست آمده پس از کشت، توسط PHA تحریک و سطح سایتوکاینهای IFN-γ، IL-12 و IL-4 در مایع رویی کشت سلول به روش الایزا تعیین گردید. تحلیل آماری داده‌ها با استفاده از نرم افزار SPSS (version 16) صورت گرفت. جهت مقایسه مقادیر سایتوکاینها آزمون آماری پارامتریک t-test و برای مقایسه مقادیر EBNA-1 و VCA آزمون غیرپارامتریک Mann-Whitney بکار گرفته شد. همچنین برای بررسی ارتباط بین متغیرها از ضریب همبستگی پیرسون استفاده شد. 05/0P < معنی‌دار در�نظر گرفته شد. </h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">یافته‌ها: میانگین سطح آنتی‌بادی‌های ضد EBNA-1 و VCA در بیماران مبتلا به MS نسبت به افراد سالم افزایش معنی‌داری داشت (به ترتیب ٠٤/٠=P و ٠٠١/٠=P). میانگین غلظت IFN-γ، IL-12 و IL-4 نیز در بیماران به طور معنی‌داری بیش از افراد سالم بود ( به ترتیب ٠٠١/٠=P‌، ٠٠٢/٠=P و ٠٠٥/٠=P). همچنین نسبت سایتوکاین‌های IL-4/ IFN-γو IL-4/ IL-12 در بیماران بیش از افراد سالم بود ولی این افزایش معنی‌دار نبود (٠٥/٠P>). بین سطح آنتی‌بادی‌های ضد EBNA-1 و VCA با تولید IL-12 ارتباط معنی‌داری وجود داشت (به ترتیبP ٠٢/٠=٢٧/٠‌= و ٠٤/٠=P، ٢٥/٠=r). در حالیکه ارتباطی میان افزایش سطح آنتی‌بادی‌های ضد اپشتاین بار ویروس با تولید سایتوکاین‌های IFN-γ و IL-4 در بیماران مشاهده نشد. </h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl">نتیجه‌گیری: با توجه به ارتباط معنی‌دار بین سطح آنتی‌بادی‌های ضد ویروس اپشتاین بار با تولید IL-12 در بیماران مبتلا به MS و نقش این سایتوکاین در هدایت پاسخ‌های ایمنی به سمت التهاب و سلول‌های TH1، این یافته‌ها پیشنهاد می‌کند که ویروس اپشتاین بار می‌تواند نقش مهمی در اتیولوژی بیماری MS ایفا کند. </h4></h2> Background & Aim: Multiple sclerosis(MS) is the most common inflammatory disease of central nervous system which is caused by an autoimmune process leading to destruction of myelin sheath. In this disease, CD4+ T–lymphocytes, mostly of TH1 phenotype, play important roles in destruction of neuronal tissues. Because of the probable etiologic participation of EBV virus in immunopathogenesis of MS, the aim of this study was to evaluate the relationship between serum levels of anti-Epstein Barr virus antibodies and production of TH1 and TH2 cytokines.Patients and Method: A descriptive study was performed on 68 MS patients in different stages of the disease and 20 healthy individuals. Blood samples were taken and plasma levels of anti-EBNA-1 and VCA antibodies were determined by ELISA method. Then, peripheral blood mononuclear cells (PBMC) were isolated by Ficoll Hipaque separation and stimulated with PHA in optimal culture conditions. Concentrations of IFN-γ, IL-12 and IL-4 in culture supernatants were then measured by ELISA. Statistical analysis was performed by using SPSS version 16. Mann-Whitney, t-test,and Pearson correlation coefficient were used to compare the variables and find the correlation between them. P<0.005 was considered statistically significant. Results: The mean levels of anti EBNA-1 and VCA antibodies were significantly higher in patients compared to controls (P=0.04, P=0.001 respectively). Concentrations of IFN-γ, IL-4 and IL-12 were also significantly higher in MS patients than healthy individuals (P=0.001, P=0.005, p=0.002 respectively). Although the ratios of IFN-γ to IL-4 and IL-12 to IL-4 were higher in MS patients than healthy individuals, this increase did not reach a significant level (P>0.05). A statistically significant correlation was found between anti EBNA-1 and VCA antibodies and IL-12 production (P =0.02, r=0.27& P=0.04, r=0.25 respectively) whereas no significant correlation was found between these antibodies and production of IFN-γ and IL-4 in MS patients. Conclusion: The significant correlation between the level of anti-Epstein Barr virus antibodies and IL-12 production in MS patients implies the probable role of this cytokine in skewing immune responses toward TH1 phenotype. It is possible that this putative viral agent in combination with other etiological agents is involved in the etiology and progression of the disease.
