fa بررسی علل زمینه‌ای تأخیر در ارجاع اطفال مبتلا به عفونت‌های سیستم ادراری به بیمارستان های سینا و بهرامی از سال 1380 تا 1386 ارولوژی Urology پژوهشي Research <h2 style="MARGIN: 0cm 0cm 0pt DIRECTION: rtl" dir="rtl" align="right"><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl" align="right"><font size="2"><span></span></font></h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl" align="right"><font size="2"><span>    زمینه و هدف: عفونت‌های سیستم ادراری، بیماری‌های شایعی هستند و ازاین بین پیلونفریت حاد شایع‌ترین بیماری عفونی کلیه می‌باشد. این دسته از عفونت‌ها، 1/1 میلیون مراجعه پزشکی در سال در ایالت متحده را شامل می‌شوند. هزینه درمانی در مواردی که نیاز به بستری باشد، بالغ بر 180 میلیون دلار در سال است. اگرچه موربیدیته این دسته بیماری‌ها و مورتالیته ناشی از آن بالاست، در صورت درمان به موقع می‌توان از عوارض برگشت ناپذیر آن مانند اسکار کلیه و نارسایی مزمن کلیه - که هزینه بالایی جهت درمان‌هایی که اغلب </span><font face="Times New Roman"><span dir="ltr">Curative</span><span dir="rtl"></span></font><span style="FONT-FAMILY: "><span dir="rtl"></span> <span lang="AR-SA"><font face="B Yagut">نیست، در برخواهد داشت- جلوگیری کرد. </font></span></span></font></h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl" align="right"><font size="2"><span style="FONT-FAMILY: " lang="AR-SA">این مطالعه با هدف شناسایی علل زمینه‌ای<font face="B Yagut"><span style="mso-spacerun: yes">  </span>تأخیر ارجاع کودکان مبتلا به عفونت‌های سیستم ادراری به مراکز درمانی طراحی شده است</font></span><span style="FONT-FAMILY: " lang="FA">، </span><span style="FONT-FAMILY: " lang="AR-SA">تا بتوان با شناسایی این<font face="B Yagut"><span style="mso-spacerun: yes">  </span>علل ، راهکارهای مناسب جهت پیشگیری از آن‌ها و لذا پیشگیری ازعوارض برگشت ناپذیر ناشی از درمان تأخیری، دست یافت و بتوان هزینه درمانی تحمیل شده به فرد و جامعه را کاهش داد. </font></span></font><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl" align="right"><font size="2"><span>روش بررسی: این مطالعه به صورت </span><font face="Times New Roman"><span dir="ltr">Case series</span><span dir="rtl"></span></font><span style="FONT-FAMILY: "><span dir="rtl"></span> <span lang="AR-SA"><font face="B Yagut">طراحی شده است. جامعه هدف رااطفال مبتلا به عفونت‌های سیستم ادراری مراجعه‌کننده به بیمارستان‌های سینا و بهرامی تشکیل می‌دهند. معیارهای ورود شامل 1- سن بین 10-1 سال 2- وجود 1 کشت ادرار مثبت در پرونده و 3- عدم وجود ناهنجاری‌های سیستم ادراری می‌باشد. از بین افرادی که صلاحیت شرکت در مطالعه را داشتند، کلیه متغیرهای مورد بحث مانند سن، جنس، میکروارگانیسم ایجاد‌کننده وغیره مورد بررسی قرار گرفت. داده‌های به دست آمده توسط </font></span></span><font face="Times New Roman"><span dir="ltr">SPSS V. 11.5</span><span dir="rtl"></span></font><span style="FONT-FAMILY: " lang="AR-SA"><span dir="rtl"></span> آنالیز شد. داده‌های کمی به صورت میانگین و انحراف معیار و داده‌های کیفی به صورت فراوانی نمایش داده شدند. برای مقایسه داده‌ها از تست</span><span style="FONT-FAMILY: " lang="FA">‌های </span><span style="FONT-FAMILY: " lang="AR-SA">آماری </span><font face="Times New Roman"><span dir="ltr">Chi2</span><span dir="rtl"></span></font><span style="FONT-FAMILY: " lang="AR-SA"><span dir="rtl"></span> و </span><font face="Times New Roman"><span dir="ltr">t test</span><span dir="rtl"></span></font><span style="FONT-FAMILY: "><span dir="rtl"></span> <span lang="AR-SA"><font face="B Yagut">استفاده شد. حد معنی‌داری 05/0 در نظر گرفته شد. </font></span></span></font><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl" align="right"><font size="2"><span>یافته‌ها: در بین 185 کودک مورد مطالعه با طیف سنی 1 الی 10 سال و میانگین سنی 4/5 سال، 57 % دختر و 43% پسر بودند. در بین 185 کودک مورد مطالعه، 9/15 % با علائم سپسیس و 2/2 % مورد با علامت شوک سپتیک مراجعه کردند. دیگر نتایج مورد بررسی به تفصیل در جداول گنجانده شده است. </span><span>نتیجه‌گیری: نهایتاً آموزش صحیح والدین از طریق آموزش حین بارداری، آموزش از طریق رسانه‌های گروهی و شناسانیدن علائم خطر و عوارض تأخیر ارجاع اطفال مبتلا به عفونت سیستم ادراری به مراکز درمانی، می‌تواند به میزان زیادی از این تأخیر پیشگیری کند و لذا با درمان به موقع این بیماری‌ها و اجتناب از عوارض آن ها </span><span dir="rtl" lang="AR-SA">هزینه سنگین ناشی از مرگ ومیر آن‌ها را کاهش داد.</span></font> </h4><h4 align="right"><p align="right" ><h4 align="right"><h4 align="right"><h4 align="right"><h2 align="right" ></h4></h4></h4></h4></h4></h4></h2> <p></p><p>  <strong>Background and Aim:</strong> Urologic infections are common infections, among which pyelonephritis is the most common. Only in United States of America they cause more than 1.1 million patient referral annually. Treatment costs for these infections are about 180 million dollars a year. Although mortality and morbidity caused by theses diseases are high, but the treatment, if done on time, is not hard and could prevent from irreversible complications such as chronic renal failure and scar. </p><p>  The aim of this study was to identify the reasons of delayed referral and underlying risk factors for this delay in children with urinary tract infection, in order to find a solution and decrease the associated complications and cost fees. </p><p> <strong> Patients and Methods: </strong>This study is a case series study. Our target groups were children with urinary tract infections that had referred to Sina and Bahrami Hospitals. Inclusion criteria included: 1- age between 1-10 years, 2- no functional complications of urinary tract, 3- a positive urine culture. </p><p>  Then questioned variants (age, sex, pathogen, etc) were gathered and later analyzed and the results were reported. To analyze our data we used SPSS V. 11.5. Descriptive data were shown as mean and standard deviation. To compare the data we used Chi² and t test. p= 0.05 was considered as meaningful. </p><p> <strong> Results: </strong>Among our 185 patients with age range of 1 to 10 years (mean age of 5.4 yr), 57% were females and 43% were male. Meanwhile 15.9% of our patients had referred to hospital with symptoms of sepsis and 2.2% with septic shock. Our results did not suggest sex as a risk factor for referral delay. We observed that 2.2% of the children in our study were hospitalized with septic shock.
