مقایسه آتراکوریوم، لیدوکایین هر یک به تنهایی و همراه با یکدیگر در پیشگیری از میالژی بعد از اعمال جراحی ناشی از ساکسنیل کولین

<p></p><p> زمینه و هدف: ساکسنیلکولین، به دلیل مزایای متعدد(ارزان بودن، شروع سریع و عدم نیاز به داروهای آنتیکولین استراز جهت از بین بردن اثر شل کنندگی آن و ...) جهت تسهیل لولهگذاری داخل تراشه خصوصاً در بیماران سرپایی بسیار مناسب میباشد؛ با این وجود، این دارو دارای تعدادی عوارض نامطلوب مانند فاسیکولاسیون عضلات و میالژی بعد از عمل جراحی میباشد و این خود باعث بروز مشکلاتی برای بیماران بخصوص بیماران سرپایی میگردد. هدف از این مطالعه، بررسی و تعیین اثر لیدوکایین وآتراکوریوم به تنهایی و یا به همراه یکدیگر در پیشگیری میالژی بعد از عمل ناشی از ساکسنیلکولین در بیماران سرپایی بود. روش بررسی: در این بررسی که به صورت آیندهنگر و علمی ـ کاربردی انجام شد، 80 بیمار در محدوده سنی 55-20 سال از بین بیمارانی که از نظر ASA(American society of Anesthesiology) در کلاس І و ІІ قرار داشتند و جهت اعمال جراحی کوچک و یا سرپایی و به صورت الکتیو در بیمارستان امام خمینی ارومیه پذیرش شده بودند، انتخاب شدند. بیماران به صورت تصادفی به یکی از چهار گروه درمانی تقسیم شدند: گروه اول، پیشدارو دریافت نکردند، گروه دوم، آتراکوریوم(3 میلیگرم)، گروه سوم، لیدوکایین(1 میلیگرم به ازای هر کیلوگرم) و گروه چهارم، آتراکوریوم(3 میلیگرم) و لیدوکایین(1 میلیگرم به ازای هر کیلوگرم) قبل از تزریق ساکسنیل کولین(5/1میلیگرم به ازای هر کیلوگرم) دریافت کردند. شیوع فاسیکولاسیون درحین القای بیهوشی، بررسی و شدت آن به وسیله Four-Point Rating Scale درجهبندی شد، همچنین شدت میالژی به وسیله Four-Point Rating Scale در ساعتهای 4، 12 و 24 بعد از عمل، بررسی وتعیین گردید. برای هر بیمار، دادههای جمعآوری شده که شامل مشخصات دموگرافیک(سن، جنس و طول مدت جراحی)، وجود فاسیکولاسیون و شدت آن، میالژی و شدت آن، زمان شروع میالژی، ارتباط فاسیکولاسیون و میالژی و میزان محدودیت فعالیت و نیاز به آنالژزی اضافی در گروههای مختلف بود، بررسی و اختلاف بین آ نها با square test-Chi وfisher’s -exact test، آنالیز و P value<0.05 با اهمیت تلقی شد. یافتهها: در هیچ یک از بیماران، عوارض ناشی از داروهای مورد مطالعه دیده نشد. شیوع و شدت فاسیکولاسیون در بیماران گروه 3 و1 نسبت به گروه 4 و2، بیشتر بود(30% و50% در مقابل 10% و10%)، البته ایــن اختــلاف تنها مابین گروه 2 و 1 (014/0P value=) و گروه 4 و1(014/0P value=) معنیدار بود، ولی این مقایسه بین سایر گروهها معنیدار نبود، یعنی میزان فاسیکولاسیون به وسیله آتراکوریوم و لیدوکایین به همراه آتراکوریوم در مقایسه با ساکسنیل کولین بدون پیشدارو، کاهش چشمگیری داشت. در مورد میالژی، 70% بیماران دارای فاسیکولاسیون، دچار میالژی بعد از عمل شدند و در 30% مابقی، میالژی بعد از عمل ظاهر نشد. مقایسه گروههای دارویی از نظر بروز میالژی در بیماران نشان میدهد که شیوع و شدت میالژی در گروه 1و3(به ترتیب 40% و25%) بیشتر از گروه 2(5%) بود و این اختلاف مابین گروه 1و2معنیدار بود(021/0=P value) ولی ما بین گروه 1و3 معنیدار نبود(5/0=P value) و در گروه چهارم نیز هیچ موردی از میالژی دیده نشد. از نظر شدت میالژی تنها دو نوع خفیف و متوسط مشاهده شد که در گروه 3و1، هر دونوع و در گروه 2، تنها نوع خفیف بروز نمود ولی هیچ مورد شدیدی گزارش نشد. نتیجهگیری: این بررسی نشان میدهد که استفاده از آتراکوریوم به مقدار 3 میلیگرم و یا لیدوکایین با دوز 1 میلیگرم به ازای هر کیلوگرم وزن قبل از تزریق ساکسنیل کولین باعث کاهش فاسیکولاسیون و میالژی بعد از عمل میگردد، که در هر دو مورد، آتراکوریوم موثرتراز لیدوکایین میباشد. همچنین این بررسی نشان میدهد، در صورتی که این دو با یکدیگر بکار روند باعث کاهش بیشتر فاسیکولاسیون شده و میتوانند از میالژی پیشگیری نمایند. </p>
<p></p><p> Background & Aim: Succinylcholine has obvious advantages for facilitating endotracheal intubation in the ambulatory setting patients(eg: low cost, fast onset and no need for reversal of neuromuscular relaxant). Unfortunately postoperative myalgia(POM) may frequently occur after the use of succinylcholine and this myalgia may be particularly troublesome in outpatients. This study was conducted to evaluate the efficacy of Atracurium or Lidocaine alone or together for prevention of succinylcholine – induced fasciculation and postoperative myalgia in patients undergoing small or ambulatory surgery. Patients and Methods: This study was prospective and clinical trial and conducted on 80 ASA physical status I, II patients aged between 30-50 years that were scheduled for elective and ambulatory or small surgeries under general anesthesia in Uromieh Imam Khomeini hospital. Patients were randomly assigned to one of the four groups(n= 20 for each group) group 1: without pretreatment, group 2: 3mg atracurium, group 3: 1mg/kg lidocaine, group 4: 3mg atracurium and 1mg/kg lidocaine together, before 1.5mg/kg succinylcholine was administered. During induction of anesthesia incidence of fasciculation was assessed and four point rating scale was used for its severity assessment. In addition, severity of myalgia was assessed by a four point rating scale at 4, 12, and 24 hours postoperatively. For each of patients data collected included demographic variables(eg: age, sex, duration of operation), presence or absence of fasciculation, myalgia, severity of myalgia, severity of fasciculation and time of onset of myalgia. Correlations between fasciculation and myalgia, limitation in ambulation, and need for excessive analgesics in postoperative phase were assessed and then differences analyzed via Fisher's-Exact test and Chi-Square test. P value<0.05 was considered significant. Results: Non of the patients complained of any side effects after administration of the study drugs. Severity and rate of fasciculation was observed less frequently in groups 1, 3 (30%,50%) as compared with groups 2,4(10%,10%). This difference was significant between groups 1 and 2(pv=0.014) and between groups 1 and 4(pv=0.014). Meanwhile this comparison was not significant between other groups. Inother words decrease in incidence of fasiculation is more pronounced with use of atracurium alone or atracurium with lidocaine than use of succinylcholine without any pretreatment. Seventy percent of fasciculated patients had myalgia in the remaining 30% postop myalgia did not occur. Incidence of myalgia was more frequent in groups 1,3(40%, 25%) as compared with group 2(5%) and this difference was significant between groups 1,2(pv=0.021). However it was not statistically significant between groups 1 and 3(Pv=0.5). However none of the patients in group 4 reported myalgia. Severity of myalgia in groups 1 and 3 was mild and moderate but this severity in group 2 was only mild. Severe myalgia was not reported in any of the cases. Conclusion: This study suggests that use of succinylcholine in conjuction with atracurium(3mg) or lidocaine(1mg/kg) as pretreatment resulted in reduction of fasciculation and postoperative myalgia, (although atracurium was more effective than lidocaine).This study demonstrated that pretreatment with atracurium combined with lidocaine was more effective in reducing Succinylcholine-induced fasciculation and postoperative myalgia, in comparison to using each alone. </p>
