بررسی مقایسهای اسیدهای چرب اشباع و ترانس و سیس در بافت چربی و سرم بیماران مبتلا به دیابت نوع 2 مراجعه کننده به انستیتو غدد داخلی و متابولیسم
A Comparative Study of Saturated, Trans and Cis Fatty Acids Measured in Serum and Adipose Tissue between Patients with Type 2 Diabetes and Normal Subjects

<p></p><p style="DIRECTION: rtl"> زمینه و هدف: دیابت یک بیماری هتروژن است که در اثر واکنشهای پیچیدهای که بین عوامل ژنتیکی، تغذیه و شیوه زندگی رخ میدهد، بوجود میآید. مطالعات انجام شده نشان میدهد که افزایش مصرف اسیدهای چرب اشباع(SFA=Saturated fatty acid)، باعث افزایش خطر ابتلا به دیابت نوع 2 میشود، در حالی که اسیدهای چرب با چند پیوند غیر اشباع(PUFA=Poly unsaturated fatty acid) از طریق افزایش قدرت اتصال انسولین به رسپتور، باعث کاهش بروز دیابت میشوند. در این مطالعه، با بررسی بافت چربی به عنوان بیومارکر مصرف طولانی مدت اسیدهای چرب و همچنین تعیین ترکیب اسیدهای چرب سرمی، ارتباط اسیدهای چرب مصرفی با ابتلا به دیابت بررسی شد. روش بررسی: این بررسی از نوع مطالعات مقطعی بوده که در آن، دو گروه افراد سالم و بیمار، مورد مطالعه قرار گرفتهاند. ترکیب اسیدهای چرب سرم ناشتا و بافت چربی در 98 بیمار مبتلا به دیابت نوع 2 و 76 فرد سالم، با روش کروماتوگافی گاز ـ مایع آنالیز شد. لیپیدهای سرمی با دستگاه اتوآنالایزر اندازهگیری شدند. دو گروه مورد مطالعه از نظر توزیع زن و مرد و نیز سن، یکسان گردیدند. مقایسه میانگینهای متغیرها در دو گروه مورد مطالعه با استفاده از آزمون t-student انجام شد. یافتهها: درصد اسیدپالمیتیک و ایزومر مکانی اسید اولئیک(18:1-c11) در بافت چربی بیماران، بیشتر از گروه کنترل بود(به ترتیب 01/0=P و 02/0=P). درصد اسید پالمیتیک، اسیدهای چرب اشباع(SFA) تام و اسیدهای چرب با یک پیوند دوگانه(MUFS=Mono unsaturated fatty Acid) تام در سرم بیماران، بیشتر از گروه کنترل بود(به ترتیب 001/0=P، 006/0=P و 02/0=P)، درصد اسید لینولئیک و اسیدهای چرب با چند پیوند غیراشباع(PUFA) تام در سرم بیماران نیز، کمتر از گروه کنترل بود(به ترتیب 02/0=P و 02/0=P). متوسط غلظت تری گلیسرید(TG=Triglyceride) در گروه بیمار، بیشتر از گروه کنترل بود(001/0=P، 7/6-=t). بین اسیدهای چرب PUFA سرم و سرم، رابطه منفی و بین اسیدهای چرب اشباع سرم و TG سرم، رابطه مثبت بدست آمد. همچنین بین اسیدهای چرب PUFA سرم و TG سرم، رابطه منفی حاصل شد. نتیجهگیری: بالا بودن اسیدهای پالمیتیک و 18:1-c11 در بافت چربی، ممکن است خطر ابتلا به دیابت نوع 2 را افزایش دهد و به نظر میرسد بیماران دیابتی با مصرف بیشتر اسیدهای چرب PUFA نسبت به اسیدهای چرب اشباع، میتوانند کنترل مناسبی بر پارامترهای لیپیدی خود داشته باشند. </p>
<p></p><p> Background & Aim: Diabetes is a heterogenous disease which results from complex reactions among heredity, nutrition and lifestyle. Some studies have shown that a high intake of saturated fatty acids(SFA) increases the risk of type 2 diabetes, while polyunsaturated fatty acids(PUFA) decrease diabetes incidence by increasing insulin affinity to the receptors. In this study, serum fatty acids and adipose tissue composition as a long-term biomarker for fatty acids intake are determined, and their correlation with type 2 diabetes is investigated. Patients & Method: This was a cross-sectional study in which the healthy and the patient groups were compared. The fatty acid composition of fasting serum and adipose tissue was studied in 98 patients with type 2 diabetes and 76 healthy control subjects using gas-liquid chromatography. The serum lipids were measured by autoanalyzer. The means of variables were compared by using Students’ t-test. Results: The percentages of palmitic acid and positional isomer of oleic acid(11c-18:1) in adipose tissue of the patients were higher than the control group(P=0.01, P=0.02 respectively). The percentages of palmitic acid, total saturated and monounsaturated fatty acids in the serum of the patients were higher than the control group too(P=0.001, P=0.006, P=0.02 respectively). Linoleic acid and polyunsaturated fatty acids were lower than the control group, however(P=0.02, P=0.02 respectively). Mean concentration of triglyceride in the patients was higher than the control group(t=-6.7, P=0.001). There was a negative correlation between serum PUFAs and cholesterol to HDL ratio in serum and a positive correlation between serum PUFAs and TG in serum. Also, PUFAs in serum had a negative correlation with TG in serum. Conclusion: Large amounts of palmitic acid and 11c-18:1 in adipose tissue may increase the risk of type 2 diabetes and it seems that patients with type 2 diabetes can have proper control over lipid parameters by having a higher intake of polyunsaturated fatty acids than saturated fatty acids. </p>
