بررسی ارتباط نتایج آزمون مونوفیلامنتهای سمز ـ وینشتاین و مطالعات هدایت عصبی در زنان مبتلا به سندرم تونل کارپ

سندرم تونل کارپ(Carpal Tunnel Syndrome=CTS) یکی از شایعترین نوروپاتیهای فشاری است که تعداد زیادی از بیماران به علت آن به مراکز کاردرمانی مراجعــه میکنند بنابراین برای ارزیابی این بیماری استفاده از آزمونهای حساس و تکرارپذیر ضــروری میباشد. در تشخیص نهایی بسیاری از بیماریها از جمله سندرم تونـــل کارپ، به دلیـل وقفهای که در هدایـــت الکتریکـــی عصــب رخ میدهد، از آزمون مطالعات هدایت عصبی(Nerve Conduction Study=NCS) استفاده میشود. از سوی دیگر به علت وجود فشار روی عصب مدین در تونل کارپ، رشتههای عصبی مختلف از جمله اعصاب مربوط به حس لمس و فشار آسیب میبینند. این رشتههای عصبی به دلیل داشتن میلین و قطر زیاد بیش از سایر رشتهها در معرض آسیب قرار دارند. از این رو آزمونهای آستانهای که آزمون مونوفیلامنتهای سمز ـ وینشتاین(Semmes Weinstein Monofilaments=SWMs) نیز از جمله آنها محسوب میگردد، حساسترین آزمونهای نشان دهنده ناهنجاری بالینی در نوروپاتی فشاری هستند. در این پژوهش سعی شد به بررسی این دو آزمون پرداخته شود سپس رابطه آنها مورد تجزیه و تحلیل قرار گیرد تا در صورت همسو بودن نتایج، بتوان از آزمون SWMs به عنوان آزمونی مکمل بهره گرفت. در این مطالعه 33 زن مبتلا به CTS به طور غیرتصادفی انتخاب شدند و آزمون NCS توسط فرد متخصص و آزمون SWMs توسط آزمونگر برای آنها انجام شد.این بررسی نشان داد که ارتباط معنیداری بین نتایج آزمون SWMs انگشتان و NCS شامل آزمونهای سرعت هدایت حسی، زمان تأخیر انتهایی حسی و حرکتی، آمپلی تود حسی و حرکتی وجود دارد که این ارتباط در انگشت شست از هم بستگی بیشتری برخوردار بود و با افزایش شدت بیماری نمره SWMs افزایش مییافت. با توجه به نتایج به دست آمده در این جامعه آماری میتوان به دلیل هزینه کمتر، در دسترس بودن و راحتی بیشتر بیمار، از آزمون SWMs به ویژه در انگشت شست استفاده کرد اما با توجه به حساسیت و ویژگی هر دو آزمون بهتر است در مواردی که ارزیابی و تشخیص دقیق یا جراحی مطرح میشود از هر دو آزمون استفاده کرد. <br>
Carpal tunnel syndrome(CTS) is the most common compression neuropathy. There are many methods for evaluation of CTS. Nerve conduction study(NCS) and Semmes-Weinstein monofilament(SWMs) test are sensitive tests for CTS evaluation, but the correlation between two methods has not been evaluated. The purpose of this study was to study the relationship between the results of SWMs test and NCS on women with CTS. Samples were 56 hands in 33 women with CTS at age of 20-73 years. SWMs test and measurement of secondary variables was done by an occupational therapist and NCS by a physiatrist. Our research method was analytical nonintervention. Variables were: age, BMI, hand length, hand width, length of distal phalanx of thumb, index and middle fingers, distal sensory latency, distal motor latency, sensory and motor amplitude, nerve conduction velocity(NCV), Semmes-Weinstein monofilament and the results of three fingers severity. Data were analyzed by simple regression test and one-way variance analysis. There was a significant relationship between results of SWMs test and NCV(P=0.0002), distal sensory latency(DSL)(P<0.00001), amplitude of sensory(Amps) NCV(P<0.00001), distal motor latency(DML)(P<0.00001) and amplitude of motor NCV(P=0.0042) which was most obvious in the thumb. There was a negative correlation between SWMs test and NCV, Amps, Ampm and there was a positive correlation between SWMs test and DSL, DML. There was a significant positive correlation between SWMs test and severity this positive correlation was more significant in the thumb than in the other two fingers. Hand dominance did not have any significant correlation with SWMs test and NCS. There is more significant relationship between SWMs test and NCS in evaluation of the first finger which is most probably because the first finger has large area in brain mapping.
