تأثیر 8 هفته تمرین در آب با دو شدت مختلف بر روی سطوح سرمی NGF و GDNF در زنان مبتلا به مولتیپل اسکروزیس
Effect of Aquatic Exercise with Two Different Intensities on Serum Levels of GDNF and NGF in Women with Multiple Sclerosis

<strong>مقدمه:</strong> از عوامل نروتروفیک مؤثر در پاتوژنز مولتیپل اسکلروزیس، عوامل رشد عصبی (<span dir="LTR">NGF</span>) و نروتروفیک مشتق از سلول های گلیال (<span dir="LTR">GDNF</span>) می باشند. با این حال، شدت تمرین در آب بر روی عوامل <span dir="LTR">NGF</span> و <span dir="LTR">GDNF</span> در بیماران مبتلا به مولتیپل اسکلروزیس کمتر شناخته شده است. هدف مطالعه حاضر بررسی تأثیر 8 هفته تمرین در آب با دو شدت مختلف (شدت پایین در مقابل شدت بالا) بر روی سطوح سرمی <span dir="LTR">NGF</span> و <span dir="LTR">GDNF</span> در زنان مبتلا به مولتیپل اسکروزیس بود.<br>
<strong>روش ها:</strong> در یک مطالعه نیمه تجربی، 35 زن مبتلا به موالتیپل اسکروزیس از نوع <span dir="LTR">RR</span> (3<span dir="LTR">EDSS<</span>، شاخص توده بدنی: 22/3±03/27) بصورت تصادفی در سه گروه تمرین در آب با شدت پایین (<span dir="LTR">LI</span>، 10 نفر)، گروه تمرین در آب با شدت بالا (<span dir="LTR">HI</span>، 14 نفر) و یا گروه کنترل (11 نفر) قرار گرفتند. برنامه تمرین شامل تمرین هوازی در آب با شدت پایین (70%-50% ضربان قلب ذخیره بیشینه) یا شدت بالا (85%-70% ضربان قلب ذخیره بیشینه) با حجم برابر، 3 بار در هفته ، 30 تا 60 دقیقه در هر جلسه برای مدت 8 هفته بود. از گروه کنترل خواسته شده الگوی زندگی روزمره طبیعی خود را در طول مدت مطالعه حفظ کنند. سطوح سرمی <span dir="LTR">NGF</span> و <span dir="LTR">GDNF</span> در حالت پایه و در پایان مطالعه اندازه گیری شد. داده ها با استفاده از تحلیل واریانس با اندازه گیری تکراری مورد تجزیه و تحلیل قرار گرفت.<br>
<strong>یافته ها:</strong> پس از 8 هفته تمرین در آب، سطوح سرمی <span dir="LTR">NGF</span> و <span dir="LTR">GDNF</span> به ترتیب 18% و 16% در گروه شدت پایین (05/0<span dir="LTR">p<</span>) و 8% و 13% در گروه با شدت بالا افزایش یافت. با این حال، هر دو گروه بهبود مشابهی در عوامل نروتروفیک نشان دادند و هیچ تفاوت معناداری بین گروه ها وجود ندشت. <br>
<strong>نتیجه گیری: </strong>این نتایج پیشنهاد می کند که مولتیپل اسکلروزیس می تواند منجر به کاهش سطوح سرمی نروتروفیک شود. به عبارت دیگر، تمرین در آب در دو دامنه با شدت های 70%-50% و 85%-70% ضربان قلب ذخیره بیشینه می تواند سطوح سرمی <span dir="LTR">NGF</span> و <span dir="LTR">GDNF</span> در زنان مبتلا به مولتیپل اسکروزیس بطور یکسان بهبود بخشد. <div style="text-align: left;"></div>
<strong>Abstract</strong><br>
<strong>Background: </strong>Nerve growth factor (NGF) and Glial cell line‐derived neurotrophic factor (GDNF) are neurotrophic factors for multiple sclerosis (MS) <a href="https://www.sciencedirect.com/topics/medicine-and-dentistry/pathogenesis" title="Learn more about Pathogenesis">pathogenesis</a>. However, the impact of aquatic exercise intensitie on NGF and GDNF factors is less clear in patients with MS. The purpose of the present study was to examine the effects of 8-weeks supervised aquatic exercise of different intensities (high- intensity (HI) versus low-intensity (LI)) on serum levels of GDNF and NGF in women with multiple sclerosis (MS).<br>
<strong>Methods:</strong> In this semi-experimental, 35 women with MS (mean age [X̅±SD]=37±9 years) were randomly assigned to either low-intensity aquatic exercise group (LI: n = 10), high-intensity aquatic exercise group (HI: n = 14), or the control group (CON: n= 11). Exercise programs consisted of aquatic aerobic exercises at either LI (50% - 70% of HRR) or HI (70% - 85% of HRR), of equal volume, 3 times per week, 30 to 60 minutes per session for 8 weeks. CON group were asked to maintain normal daily life pattern for the duration of the study. Serum levels of GDNF and NGF were measured at baseline and at the end of the study. The data were analyzed using repeated-measures analysis of variance.<br>
<strong>Findings:</strong> After aquatic exercise, the serum NGF and GDNF levels increased by 18% and 16% in the LI group (P < .05) and by 8% and 13% in the HI group (P < .05) compared with baseline. However, both groups showed similarly improved neurotrophic factor variables and there was no difference across the groups.<br>
<strong>Conclusion:</strong> These results suggest that MS can reduce serum neurotrophic factor levels. On the other hand, aquatic aerobic exercise of intensities (50% - 70% or 70% - 85% of HRR) can similarly improve serum NGF and GDNF levels imbalance in overweight women MS.<br>
