تغییرات هدایت استخوانی پس از بازسازی زنجیره استخوانچهای گوش میانی

<p></p><p> زمینه و هدف: شایعترین علت کاهش شنواییهای قابل اصلاح با جراحی، بیماریهای گوش میانی است که در میان آنها، اختلال عملکرد زنجیره استوانچهای، بیشترین تاثیر را بر شنوایی بیمار دارد. بازسازی موفق این زنجیره، قطعاً با بهبود آستانههای هدایت هوایی و بستهشدن air-bone gap در رادیوگرام همراه خواهد بود. اما اثر این بازسازی بر روی هدایت استخوانی، که از نظر فیزیولوژیک قابل انتظار است، هنوز مشخص نشده است و تنها تعداد اندکی مطالعات استاندارد در قالب کارآزمایی بالینی، در این زمینه گزارش شدهاند. اغلب مطالعات قبلی به صورت گذشتهنگر بوده و با خطاهای متدولوژیک همراهند. هدف از این مطالعه، تعیین میزان تاثیرپذیری هدایت استخوانی از بازسازی زنجیره استخوانچهای میباشد. روش بررسی: در یک کارآزمایی بالینی کنترل شده، آستانههای هدایت استخوانی در فرکانسهای گفتاری، در 68 بیمار قبل و بعد از اعمال جراحی گوش میانی مورد مطالعه قرار گرفته است. به منظور تعیین میزان تاثیر بازسازی زنجیره استخوانچهای بر روی هدایت استخوانی، بیماران در 2 گروه و هر گروه شامل 34 بیمار که در یک گروه بازسازی زنجیره استخوانچهای در حین عمل جراحی انجام شده است و در گروه دیگر این بازسازی به مرحله دوم موکول گردیده است، بررسی شدهاند. تغییرات میانگین آستانههای هدایت استخوانی در هر فرکانس، قبل از عمل و بعد از عمل، در هر گروه با استفاده از Paired-samples t-test مورد آزمون قرار گرفتهاند. یافتهها: در گروه بازسازی شده در 2 فرکانس 1000 و 2000 هرتز، بهبودی معنیدار در آستانه هدایت استخوانی مشاهده گردیده که حداکثر آن، در فرکانس 2000 و به مقدار 7/9 دسیبل است. در گروه بدون بازسازی در هیچ یک از فرکانسها تغییر معنیداری بدست نیامده است. نتیجهگیری: بهبود هدایت استخوانی پس از بازسازی، نشان دهنده نقش و جایگاه این زنجیره در این نوع هدایت است که در گسستگیهای زنجیره در بیماریهای گوش میانی، دچار اختلال میشود و پس از بازسازی موفقیتآمیز زنجیره، دوباره به حالت طبیعی باز میگردد. این یافتهها تایید کننده تغییر هدایت استخوانی به عنوان یک پدیده مکانیکی متاثر از زنجیره استخوانچهای است. به عبارت دیگر هدایت استخوانی منحصراً تحت تاثیر عملکرد کوکلئا نیست. از سوی دیگر وقوع کاهش شنوایی حسی ـ عصبی به عنوان یک عارضه جراحی گوش میانی، پدیده شایعی نیست. </p>
<p></p><p> Background & Aim: The most common surgically treatable causes of hearing loss are middle ear diseases among which the greatest effect is related to ossicular chain dysfunction. Successful ossicular chain reconstruction certainly improves air conduction thresholds and closes air-bone gap in audiograms, but the effect of this procedure on bone conduction thresholds has remained to be elucidated. Only few standard clinical trials have been done and most studies are retrospective with methodological faults. The aim of the present study was to determine the effect(s) of ossicular chain reconstruction on bone conduction thresholds. Patients & Method: In a controlled clinical trial, pre-and post-operative bone conduction(BC) thresholds of speech frequencies were investigated in 68 patients. In order to evaluate the effect of ossicular chain reconstruction on bone conduction, the patients were divided into two groups including 34 cases each. One group received ossicular chain reconstruction during the middle ear surgery and the other one was to receive it later. In each group, mean preoperative bone conduction thresholds of any frequencies were compared with postoperative ones using paired t-test. Results: In the reconstruction group, the results revealed a significant improvement in the postoperative BC thresholds at 1000 and 2000 Hz with the largest mean improvement observed at 2000 Hz(about 9.7 db recovery). In the group without reconstruction, no significant changes were observed at any frequencies. Conclusion: The results indicated the remarkable effect of ossicular chain on bone conduction. This effect is eliminated in ossicular disruptions due to various middle ear diseases and is reproducible by successful reconstruction of the chain. The findings showed changes in bone conduction as a mechanical phenomenon affected by ossicular chain. In other words, BC thresholds are not exclusive indicators of chochlear function. We also observed that true sensorineural hearing loss is not a common major complication of middle ear surgery. </p>
