<p></p><p> زمینه و هدف: اریون در ایران آندمیک است. به علت بالا بودن درصد افراد غیرواکسینه جوان(کمتر از 15 سال) در ایران، احتمالاً بروز اریون و عوارض آن در مقایسه با کشورهای توسعه یافته قبل از واکسیناسیون عمومی بیشتر است. هدف از این تحقیق تعیین نقش اریون در کری عصبی کودکان بوده است. روش بررسی: این مطالعه مقطعی توصیفی در سالهای 1382-1381 برروی 87 کودک حداکثر تا 14 سال مبتلا به کری عصبی و 30 کودک شاهد در بخش کودکان و گوش و حلق و بینی بیمارستان رسول اکرم(ص) تهران انجام گرفت. در نمونه خون کودکان، وجود IgM و IgG اختصاصی اریون به روش الیزا بررسی شد. یافتهها: میانگین سنی افراد تحت بررسی، 9/27±8/39 ماه و 6/60 درصد پسر و 4/39 درصد دختر بودند. گروه سنی 3 تا 5 سال بیشترین فراوانی(5/51%) و سن بالاتر از 10 سال کمترین فراوانی را داشتند. عفونت حاد اریون(IgM) در 3/8درصد و ایمنی قبلی به اریون(IgG) در 1/24درصد کل موارد دیده شد. بیشترین فراوانی عفونت حاد اریون(IgM مثبت) در گروه سنی 3 تا 5 سال بود و در گروههای سنی مختلف تفاوت نداشت(11/0=p). بیشترین فراوانی ایمنی قبلی اریون(IgG مثبت) در گروه سنی 5-3 سال و در گروههای سنی مختلف متفاوت بود(05/0=p). ابتلا به عفونت حاد(IgM) و قبلی اریون(IgG) به جز میانگین سن کودکان(013/0؛ 015/0=p)، در گروه بیمار و شاهد و دو جنس تفاوتی با گروه غیرمبتلا نداشت(4/0؛1=p). 75 درصد کل کودکان فاقد ایمنی به اریون بودند. 24درصد کودکان با کاهش شنوایی(مانند کودکان سالم) ایمنی به اریون داشتند. 8/8درصد کودکان 3 تا 5 سال گروه بیمار در مقایسه با 8/6درصد گروه شاهد، مبتلا به عفونت حاد اریون بودند(11/0=p) اما ایمنی قبلی به اریون در گروه بیمار کمتر از شاهد بود(4/0=p). نتیجهگیری: بنابراین عفونت اریون در کودکان 3 تا 5 سال مبتلا به کاهش شنوایی عصبی شایعتر و مهمتر از سایر سنین است. چون با افزایش سن ابتلا به اریون، احتمال بروز کاهش شنوایی عصبی هم افزایش خواهد یافت. پیشگیری از اریون با واکسن موثر و ارزان اریون کمککننده است. بنابراین با انجام واکسیناسیون وسیع اریون در جمعیت کمتر از 20 سال، میتوان هزینه ابتلا به اریون و عوارض آن را کاهش داد. </p>
<p></p><p> Background & Aim: Mumps infection is endemic in Iran. Due to high percentage of unvaccinated young people(<15yr old) in Iran, the incidence rate of mumps infection and its sequeles are probably higher in comparison with its incidence in developed countries before massive vaccination. The goal of the present study was to determine the role of mumps infection in children with sensory neural hearing loss. Patients & Methods: This descriptive cross-sectional study was conducted on 87 children aged up to 14 years with SNHL and 30 controls in the pediatric and ENT ward of Hazrat Rasool Hospital in Tehran from 2002-2003. The blood samples of these patients were tested for specific antibody(IgM&IgG) for mumps by Elisa method. Results: The mean age was 39.8+27.9 months(male: 60.6% and female 39.4%). The highest incidence of age was 3-5 years(51.5%) and the lowest one was>10y(2.1%). Acute mumps infection(positive IgM) was seen in 8.3% and previous immunity to mumps(IgG) in 24.1% of all. The highest incidence of acute infection(IgM) was in 3-5-year old group, but it was not different in other age groups(P=0.11). Previous infection(IgG was also higher in 3-5-year old group and it was significantly different in other age groups(P=0.05). Acute(IgM) and previous infection(IgG), except in mean age of children(P=0.015, 0.013), was not significantly differente in terms of sex in case and control groups(P=1 0.4). Seventy-five percent of all children had no immunity to mumps infection. Twenty-four percent of SNLH patients(like other healthy children) had immunity to mumps infection. Acute mumps infection was seen in 8.8% of 3-5-year-old children of case compared to 6.8% of control group(P=0.11) However, previous immunity in case was lower than control group(P=0.4). Conclusion: Mumps infection in SNLH children aged between 3-5-year-old is more common and important than other age groups. Since the risk of SNHL after mumps infection will increase in older age, prevention of mumps by inexpensive and very effective mumps vaccine is helpful. Thereby, the costs of mumps infection and its sequles can be decreased with massive vaccination of Iranian young people (<20 years old). 3) Mumps Vaccination </p>
