<p> <strong> زمینه و هدف: </strong>عدم تشخیص به موقع بیماریهای مادرزادی قلبی سیانوتیک و شدید ( critical ) که در بدو تولد بدون علامت هستند یکی از علل مهم مرگ و میر در نوزادان میباشد. هدف از مطالعه حاضر بررسی حساسیت و ارزش تشخیصی پالس اکسیمتری دست و پا در روز اول تولد برای کشف بیماریهای قلبی مادرزادی سیانوتیک و شدید ( critical ) در نوزادان بدون علامت میباشد. </p><p> <strong> روش کار </strong>:<strong> </strong>در<strong> </strong>این مطالعه مقطعی 1506 نوزاد ترم و سالم که در 6 ماهه اول سال 1385 در زایشگاه شبیه خوانی کاشان متولد شده بودند، تحت پالس اکسیمتری دست و پا ی راست قرار گرفتند. در صورت 95% SPO <sub>2 </sub>< در دست یا پای راست، مجدداً 2 ساعت بعد پالس اکسیمتری در دست و پای آنها انجام شده و نهایتاً نوزادان با 95% SPO <sub>2 </sub>< در مرحله دوم، جهت انجام اکوکاردیوگرافی ارجاع شدند. نتایج به دست آمده با استفاده از نرم افزار SPSS ویرایش 11 مورد تجزیه و تحلیل قرار گرفت. </p><p> <strong> یافتهها </strong>:<strong> </strong>از 1506 نوزادی که در مرحله اول تحت پالس اکسیمتری قرار گرفتند، 1406 نوزاد SPO<sub>2</sub>≥95% در دست و پای راست داشتند (میانگین20/2 ± 12/97% در دست و میانگین 13/2 ± 9/96% در پا) و 100 نوزاد 95% SPO<sub>2</sub>< در دست و پای راست داشتند (میانگین 17/3 ± 35/93% در دست و01/3 ± 41/92% درپا) که در هر دو گروه میانگین SPO<sub>2</sub> در پای راست از میانگین آن در دست راست کمتر بود. از 100 نوزادی که 95% SPO<sub>2</sub>< داشتند، برای بار دوم پالس اکسیمتری انجام شد که نهایتاً 29 نوزادی که به طور پایدار 95% SPO<sub>2</sub>< داشتند تحت اکوکاردیوگرافی قرار گرفتند. از این تعداد در 6 نوزادی که هیچگونه علائم بالینی در هنگام معاینه فیزیکی نداشتند بیماری مادرزادی قلبی سیانوتیک و شدید ( critical )، که تعدادی از آنها نیز وابسته به مجرای شریانی بودند (شیوع 4 در 1000 تولد زنده)، تشخیص داده شد. این بیماریها عبارت بودند از: جابهجایی عروق بزرگ، تنه واحد شریانی، بازگشت غیر طبیعی وریدهای ریوی و تترالوژی فالوت از نوع شدید. ارتباط معناداری بین نتیجه اکوکاردیوگرافی با SPO<sub>2</sub> پای راست در نوزادان مبتلا به بیماریهای مادرزادی قلبی وجود داشت ( 004/0 p= ). </p><p> <strong> نتیجه گیری </strong>: با توجه به اهمیت تشخیص بیماریهای مادرزادی قلبی میتوان از پالسی اکسیمتری علاوه بر معاینه فیزیکی برای تشخیص زودهنگام بیماریهای مادرزادی قلبی سیانوتیک و شدید که در بدو تولد بدون علامت هستند، کمک گرفت. از آنجایی که همواره میزان اشباع اکسیژن شریانی در هر دو مرحله پالس اکسیمتری در اندام تحتانی کمتر از اندام فوقانی بود، به نظر میرسد میتوان از همان ابتدا فقط اندام تحتانی را مورد پالس اکسیمتری قرار داد و نیازی به پالس اکسی متری دست نمیباشد. </p>
<p> <strong> Background </strong>: Delayed diagnosis of critical and cyanotic congenital heart diseases in asymptomatic newborns can lead to significant morbidity and mortality in this age group. The aim of this study was to determine the utility of hand and foot oxygen saturation as a screening test for the early detection of critical and cyanotic congenital heart diseases in asymptomatic newborns in the first day of life. </p><p> <strong> Methods: </strong><strong></strong>In this cross-sectional study during a 6 month period we performed hand and foot pulse oximetry in 1506 term and healthy newborns in Shabihkhani Maternity Hospital. Babies with functional oxygen saturation below 95% in either right hand or right foot were considered abnormal and a second saturation measurement was performed 2 hours later. If the repeated measurement was again below 95% they were referred for performing an echocardiogram. The data was analyzed by SPSS version 11. </p><p> <strong> Results </strong>: During the study period a total of 1506 newborns were screened of whom 1406 cases had SPO<sub>2</sub>≥95% in both extremities (mean %97.12±2.20 in right hand and mean %96.9±2.13 in right foot) and 100 babies had SPO<sub>2</sub><95% (mean %93.35±3.17 in right hand and %92.41±3.01 in right foot). The mean SPO<sub>2</sub> in the right foot was lower than the right hand in the both groups. The second saturation measurement was performed 2 hours later on 100 newborns with SPO<sub>2</sub> <95%, of whom 29 had persistent low saturation who were referred for echocardiography. Six asymptomatic babies out of 29 had critical and cyanotic congenital heart diseases (incidence of 4/1000 live births). These diseases were: transposition of great vessels, truncus arteriosus, total anomalous pulmonary venous return and tetralogy of fallot. There was a meaningful correlation between right foot SPO<sub>2</sub> and echocardiography results in newborns with these types of congenital heart diseases (p=0.004) </p><p> <strong> Conclusion: </strong>According to the significance of diagnosis of congenital heart disease, pulse oximetry screening along with clinical examination could help in the early detection of critical and cyanotic congenital heart diseases in asymptomatic newborns.<i> </i>Performing right foot pulse oximetry seems to be adequate for screening, because in all the circumstances it was lower than hand saturation. </p>
