بررسی فلور میکروبی مایع صفرا در بیماران مبتلا به سنگ کیسه صفرا تحت عمل جراحی کله سیستکتومی در مرکز درمانی 5 آذر شهرستان گرگان طی سال های 1385-1384

<p style="DIRECTION: rtl"> <strong> زمینه و هدف </strong>: عفونت سیستم مجاری صفراوی و کیسه صفرا اکثراً مرتبط با انسداد جریان صفرا به علت سنگ میباشد. وجود باکتری در طول مدت التهاب کیسه صفرا ثابت شده و روشن شده که طولانیتر شدن التهاب، ممکن است بر میزان عفونت بعد از عمل جراحی تأثیر بگذارد. در این تحقیق ما به دنبال تعیین فلور میکروبی صفرا در بیماران مبتلا به التهاب حاد و مزمن کیسه صفرا بودیم تا با انجام تعیین حساسیت آنتی بیوتیکی بتوان آنتی بیوتیک مناسب جهت درمان عفونت قبل از عمل را به کار برد. </p><p> <strong> روش کار </strong>: این مطالعه از نوع توصیفی - مقطعی است که در آن از کلیه بیماران مبتلا به التهاب حاد و مزمن کیسه صفرا ناشی از سنگ، تحت عمل جراحی کله سیستکتومی قرار گرفته، نمونه استریل حین عمل از کیسه صفرا تهیه و جهت بررسی به آزمایشگاه ارسال شد. نمونه ارسالی از نظر باکتریهای هوازی، بی هوازی، گرم مثبت و گرم منفی بررسی شد. کشت مایع در عرض یک ساعت پس از گرفتن نمونه انجام شد. اطلاعات به دست آمده وارد رایانه شده و با نرم افزار آماری SPSS نسخه 5/11 مورد بررسی قرار گرفت. </p><p> <strong> یافتهها </strong>: اکثریت افراد (1/73%) مبتلا به کله سیستیت مزمن بودند و بیشترین سنگ به دست آمده (7/48%) از نوع کلسترولی و سپس نوع مختلط (5/29%) بود. نوع سنگ به دست آمده و نتیجه کشت مایع صفرا هیچ ارتباط معنیداری نشان نداد. کوکسیهای گرم مثبت (مانند استرپتوکوکوس ویریدنس) و باسیلهای گرم منفی (مانند سیتروباکترفروندئی) به ترتیب 5/11% و 9/17% در این افراد دیده شدند. بیشترین حساسیت آنتیبیوتیکی به آنتیبیوتیکهای فلوروکینولون بود. </p><strong>نتیجهگیری </strong>: بررسی نتایج نشان دهنده عدم عفونی بودن مایع صفرا در اکثریت موارد کله سیستکتومی و حساسیت ارگانیزم های رشد کرده در این مایع (در موارد مثبت) به آنتیبیوتیکهایی مانند سیپروفلوکساسین، نورفلوکساسین و افلوکساسین بود. رابطه بین نوع سنگ ونتیجه کشت معنیدار نبود.
<p> <strong> Introduction: </strong>Bile duct obstruction due to gall stone is the most common cause of biliary infection. Bacteria stabilize during the inflammation of gallbladder and it is confirmed that longer duration of inflammation is associated with more risk of post operative infection. In this study we sought to determine the biliary microbial flora in patients with acute and chronic cholecystitis in order to find the suitable antibiotic to treat preoperative infection by using antibiotic susceptibility test (antibiogram). </p><p> <strong> Methods </strong>: This was a descriptive cross sectional study. From all patients with acute and chronic cholecystitis due to stones that underwent cholecystectomy a sterile sample was taken during surgery and sent for laboratory analysis. It was examined for gram negative and gram positive, aerobic and anaerobic bacteria. Bile culture was performed within an hour after taking samples. The data were registered and analyzed by statistical SPSS software version 11.5. </p><p> <strong> Results </strong>: The majority of patients (73.1%) suffered from chronic cholecystitis and the stones obtained were 48.7% of cholestrolic type and 29.5% of mixed type. No significant relationship existed between stone type and bile culture results. Gram positive cocci (e.g. streptococcus) and gram negative bacili (e.g. citrobacterfrondei) were found in 11.5% and 17.9% of the patients, respectively. Most sensitivity to antibiotics was towards fluoroquinolones. </p><p> <strong> Conclusion </strong>: Study results showed that bile fluid was not infected in most of the cases and in positive cases sensitivity to fluoroquinolones such as ciprofloxacin, ofloxacin and norfloxacin was detected. The relationship between stone type and culture results was not significant. </p>
