بررسی اثر درمانی تزریق داخل ضایعهای بلئومایسین بر زگیلهای مقاوم

<h2 style="MARGIN: 0cm 0cm 0pt DIRECTION: rtl" dir="rtl"><p><span><font size="2"><font face="tahoma,arial,helvetica,sans-serif"></font></font></span></p><p><span><font size="2"><font face="tahoma,arial,helvetica,sans-serif"> زمینه و هدف: زگیلها، تومورهای خوشخیم پوست و مخاط هستند که توسط ویروس پاپیلومای انسانی بوجود آمده و جزو بیماریهای پوستی بسیار شایعی میباشند که به خصوص در کودکان و نوجوانان به وفور یافت میشوند.</font></font></span><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl"><span style="FONT-FAMILY: " lang="FA"><font size="2"><font face="tahoma,arial,helvetica,sans-serif">هر چند که بسیاری از زگیلها خود به خود یا با درمانهای معمول بهبود مییابند، ولی موارد مقاوم، پایدار و صعب العلاج از معضلات شایع رشته در ماتولوژی میباشند. استفاده از داروی بلئومایسین به صورت تزریق داخل ضایعهای، برای درمان زگیلهای مقاوم به کار برده میشود که نتایج متغییری را در بررسیهای انجام شده، داشته است. هدف از این پژوهش، بررسی میزان تأثیر این دارو بر زگیلهای مقاوم در بیماران مورد مطالعه و مقایسه آن با مطالعات مشابه دیگر میباشد.<p></p></font></font></span></h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl"><font size="2"><font face="tahoma,arial,helvetica,sans-serif"><span>روش بررسی: این مطالعه، به صورت مقطعی- تحلیلی انجام گرفته است.مجموعاً 130 زگیل در 30 بیمار تحت درمان با بلئومایسین به روش تزریق داخل ضایعه با غلظت حدود 1 میلیگرم به ازای هر میلیلیتر </span><span dir="ltr"></span><span style="mso-bidi-language: FA" dir="ltr"><span dir="ltr"></span>(mg/ml)</span><span dir="rtl"></span><span style="FONT-FAMILY: " lang="FA"><span dir="rtl"></span> قرار گرفته و در صورت لزوم تا سه دوره هر کدام به فاصله 4 هفته تکرار شده، و تا 6 ماه پس از آخرین تزریق تحت پیگیری قرار گرفتند. نتیجه درمان بهصورت بهبودی کامل، نسبی و عدم پاسخ با توجه به محل و اندازه زگیلها مورد آنالیزبا نرمافزار آماری </span><span style="mso-bidi-language: FA" dir="ltr">SPSS V13</span><span dir="rtl"></span><span style="FONT-FAMILY: " lang="FA"><span dir="rtl"></span> و تستهای آماری</span><span dir="ltr"></span><span dir="ltr" lang="FA"><span dir="ltr"></span> </span><span dir="ltr">t test </span><span style="FONT-FAMILY: " lang="FA">و</span><span dir="ltr"></span><span dir="ltr"><span dir="ltr"></span> Anova test</span><span dir="rtl"></span><span style="FONT-FAMILY: " lang="FA"><span dir="rtl"></span> قرار گرفت.<p></p></span></font></font></h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl"><font size="2"><font face="tahoma,arial,helvetica,sans-serif"><span>یافتهها: از 130 زگیل مورد مطالعه، در 126 مورد (97%) پاسخ مشاهده شد به طوری که 95 مورد (73%) بهبودی کامل و 31 مورد (24%) بهبودی نسبی نشان دادند. میزان بهبودی کامل در زگیلهای بالاتر از 20 میلیمتر </span><span dir="ltr"></span><span style="mso-bidi-language: FA" dir="ltr"><span dir="ltr"></span>(mm)</span><span dir="rtl"></span><span style="FONT-FAMILY: " lang="FA"><span dir="rtl"></span> نسبت به انواع کوچکتر از </span><span style="mso-bidi-language: FA" dir="ltr">mm</span><span style="FONT-FAMILY: " lang="FA">20، 46% در مقابل 80% بود. 6 ماه پس از آخرین تزریق، از 30 بیمار تحت درمان 7 نفر دچار بروز زگیل در محل دیگر یا عود در محل بهبودی نسبی قبلی شدند. عود در آنهایی که تعداد زگیل بیشتری داشتند </span><span style="FONT-FAMILY: " lang="AR-SA">شایعتر</span><span style="FONT-FAMILY: " lang="FA"> بود.<p></p></span></font></font></h4><h4 style="MARGIN: 0cm 0cm 0pt" dir="rtl"><span><font size="2"><font face="tahoma,arial,helvetica,sans-serif">نتیجهگیری: تزریق داخل ضایعهای بلئومایسین، روش مؤثر در درمان زگیلهای مقاوم و پایداری است که به درمانهای معمولی پاسخ ندادهاند، مشروط بر اینکه نکات اصولی رعایت و احتیاطات لازم به عمل آمده و با غلظت و دوز مناسب تزریق گردد.<p></p></font></font></span></h4></p></h2><p></p>
<p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana"><strong></strong></font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana"><font style="BACKGROUND-COLOR: #f7f8fc" face="Tahoma"> </font><strong>Background<span style="mso-bidi-font-weight: bold"> and Aim:</span></strong> Warts are benign tumours of the skin and mucosa and are caused by Human Papilloma viruse (HPV). They are one of the common skin disorders found mainly in children and adolescents.</font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana">Although most of the warts disappear spontaneously and or respond to the usual therapeutic measures, difficult to treat and persistent forms of warts exist which are a problem in dermatology.</font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana">Intra lesional injection of Bleomycin has been used in the treatment of resistant warts, and is associated with variable results in different researches. This study assessed the efficacy of intralesional administration in resistant warts and compared it with other simillar studies.</font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana"><b style="mso-bidi-font-weight: normal">Patients and Methods:</b> This was a cross sectional study. A total of 130 warts from 30 patients were injected with (1 mg/ml) intralesional injection of Bleomycin. Patients received one to three times of injection of bleomycin every 4 weeks, and were followed for six months. Response to treatment was as complete resolution, partial resolution and not treated. We used SPSS version 13 and t and Anova tests as statistical methods for analyzing the data.</font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana"><b style="mso-bidi-font-weight: normal">Results<span style="mso-bidi-font-weight: bold">:</span></b> 126 of 130 warts (97%), showed resolution after bleomycin injections 95 warts (73%) showed complete resolution and 31(24%) showed incomplete resolution. The cure rate was better in warts with size less than 20 mm compared to those with lesions greater than 20mm ( 80% vs 46% ). After 6 months follow up, 7 of the 30<span style="mso-spacerun: yes"> </span>patients had recurrence of lesions. Recurrence was more among those with greater number of warts.</font></p><p style="MARGIN: 0cm 0cm 0pt" class="Style2"><font face="Verdana"><b style="mso-bidi-font-weight: normal">Conclusion<span style="mso-bidi-font-weight: bold">:</span></b> It is concluded that this form of treatment for resistant warts is reliable, safe and acceptable to patients, if performed undercare and with proper dose and concentration.</font></p>
