To reduce the contrast media-related risk, it is wise to use low osmolar contrast media (LOCM) in high-risk patients. When there is negligible cost difference between LOCM and other contrast media, it is wise to use LOCM for all patients. If an alternative procedure could avoid use of contrast media, it should be preferred (especially in high risk patients).
